  <p>We are looking for an energetic, highly motivated Clinical/Counselling Psychologist or CBT Therapist to join our dynamic Diabetes Psychology Team (30 hours per week).</p> <p>If you are an enthusiastic and motivated clinician with experience in working with people with diabetes or Long-Term Conditions (LTC), and you are passionate about delivering evidence-based psychological interventions and service development, we encourage you to apply.</p> <p>This is an excellent opportunity to develop advanced clinical skills within a multidisciplinary team and be involved in a key service development project. Regular supervision will be provided.</p> <p>The post holder will be responsible for delivering a specialist psychology service to patients living with Type 2 diabetes in Well Street Common PCN and Hackney Marshes PCN.</p> <p>Your work within the Diabetes Psychology team will be central to improving patient engagement, self-management, and overall outcomes, with a specific focus on addressing health inequalities. This includes identifying and working with patients who struggle to attend appointments, as well as providing one-to-one and group-based support for individuals struggling with diabetes related distress.   <ul>  <li>Clinical: Providing specialist psychological interventions to complex patients living with Type 2 diabetes, within HM &amp; WSC PCNs to improve self-management and promote active, healthy lifestyles.</li>  <li>Service &amp; Community Engagement: Identifying barriers to engagement in diabetes management within the two PCNs through focused research, including questionnaires and focus groups.</li>  <li>Awareness &amp; Outreach: Engaging with the community to raise awareness of diabetes management resources, with a strong focus on marginalised groups.</li>  <li>Staff Training &amp; Support: Skilling up existing staff on the knowledge and awareness of psychological barriers to engagement in diabetes care, and utilising resources like MDMs to improve the effectiveness of patient contact and reduce disengagement.</li>  <li>Peer Support: Setting up peer support initiatives based on identified need, making strategic use of existing community resources.</li>  <li>Sustainability: Developing a sustainable model of care that can be fully integrated into the PCN's ongoing services after the initial funding period concludes.</li> </ul>

  <p>The Children and Young People's (CYPS) Directorate in Surrey and Borders Partnership NHS Foundation Trust is offering an exciting opportunity for a Band 7 Clinical or Counselling Psychologist or CBT Therapist to join the well-established multi-disciplinary Mindworks Hub South. Mindworks Hubs are part of Mindworks Surrey, an alliance of NHS, national and local voluntary sector organisations providing mental health services to children in Surrey using the THRIVE approach. The Mindworks Hub South has the main base in Frimley, with a satellite office in Guildford.</p> <p>This post will be suitable for a passionate and highly motivated psychologist / CBT therapist interested in ensuring excellent psychological care is accessible for children and young people with complex mental health needs from diverse backgrounds, through delivering direct therapeutic interventions, providing supervision to junior colleagues, and providing consultations to the MDT and other services. This is a part-time (18.75 hours) secondment cover post based in Guildford.</p> <p>Psychology in SABP has a history of being highly valued and clinicians are well supported to develop their generic and specialist skills in a range of therapeutic models.   <p>The Perinatal Mental Health Service has a highly skilled team of perinatal psychological therapists who are committed to providing evidence-based interventions to women, birthing people and their families that are accessible and acceptable to the diverse South London population. We are recruiting a b8a Clinical/Counselling Psychologist/CBT Therapist to work in Lambeth and across the Trust as required.</p>
<p>Our psychological workforce and the therapeutic modalities that we offer are a key part of delivering the ambitions of the NHS Long Term Plan. This post focuses primarily on delivery of perinatal psychological therapies for maternal mental health problems, including Cognitive Behaviour Therapy (CBT), Eye Movement Desensitisation Reprocessing (EMDR), Interpersonal Therapy (IPT) and Dialectical Behaviour Therapy (DBT). There is a focus on coproduction, quality, supervision and ongoing audit and evaluation, keeping the experience of service users at the heart.</p>

  <h3>Main duties of the job</h3>
  <p>We are looking for a Clinical or Counselling Psychologist or Accredited Cognitive Behaviour Therapist to provide psychological assessment and intervention to women, birthing people and their families in the perinatal period, as well as contribute to the multi-disciplinary team.</p>
<p>We would like to hear from those who have experience delivering psychological therapies in secondary care or within a MDT setting, and are proficient in delivering NICE concordant interventions for depression, anxiety disorders, psychosis and bipolar affective disorder.</p>
<p>You will ideally have experience of working with women, birthing people and their families within the perinatal period and / or a keen interest to develop knowledge and skills in this area. Specialist perinatal supervision will be provided and you will join a friendly and supportive team of specialist perinatal psychologists and psychological therapists.</p>



  <h3>About us</h3>
  <p>Perinatal Mental Health Teams (PMHTs) are secondary care mental health teams that specialise in supporting women and birthing people in the perinatal period (pregnancy through until 12 months postnatal) who have severe and complex mental health difficulties.</p>
<p>We assess and treat a range of difficulties including mood disorders, anxiety disorders, psychosis, personality disorders and eating disorders. The service is designed to provide timely input to families to mitigate the impact of mental health disorder on the child.</p>
<p>We are a multi-disciplinary team including psychiatry, psychology, nursing, occupational therapy, and nursery nursing. The service works closely with maternity/obstetric services as well as other agencies (e.g., health visiting and social services). We have close links with the Mother-Baby Unit (MBU) in-patient unit at the Bethlem Royal Hospital.</p>

  <p>Windsor Primary Care Network, is a group of 6 GP
Practices working together with a range of local providers to offer more
personalised and coordinated health and social care to the 70000 patient
population of Windsor in Berkshire.</p><p>We are looking to recruit to the post of Health and
Well-being Coach, to work within our Primary Care Network multidisciplinary
healthcare team.</p>



  <h3>Main duties of the job</h3>
  <p> Provide one-to-one coaching to
help patients identify goals and take steps towards improving their health and
wellbeing.</p><p> Support patients with
lifestyle change areas such as weight management, physical activity, healthy
eating, stress management, sleep, and smoking cessation.</p><p> Use behaviour change
approaches including motivational interviewing, active listening, and goal
setting to encourage patient confidence and self-management.</p><p> Deliver coaching remotely or
face-to-face across primary care sites.</p><p> Connect individuals with peer
support, self-management tools, and community activities to build confidence,
resilience, and social connection.</p>

  <p>Windsor Primary Care Network has been successfully
delivering at scale Care to the population of Windsor for 4 years. We have a
large team of both clinical and none clinical staff.</p><p>While each Health and Well-being Coach will become embedded
in up to 3 Practices, they frequently enjoy a mutually supportive joint
approach to work with other members of the 13 strong PCN Health and Wellbeing

pro-active steps to improve the way they manage their physical and mental
health conditions, based on what matters to them. They support people to
develop their knowledge, skills, confidence in managing their health and care,

changes in health related behaviours. Health coaches do this by coaching and
motivating people through multiple sessions to identify their needs and to set
goals for themselves, and through providing access to interventions such as
self-management education and peer support.</p><p>Health and Wellbeing Coaches manage and prioritise a caseload,
according to the needs, priorities and support required by individuals in the
caseload. They identify when a persons needs are beyond the scope of the health
and wellbeing coach role and refer them back to other health professionals or
organisations. Health and Wellbeing Coaches may work with people by phone, by
video conference, or face- to-face.</p><p>The successful candidate will be kind, reflective and
self-aware and will enjoy working with a wide range of people. They will have
good communication and negotiation skills and a firm belief that people have
