  <p>The Young Person Link Worker is a vital member of the Berkeley Vale PCN team, acting as a bridge between primary care, young people, their families, and community services. This role supports young people (typically aged between 11 and 25) who present with social, emotional, and mental health concerns by providing early intervention, assessment, personalized support planning, and onward referrals.</p>



  <h3>Main duties of the job</h3>
  <p>This position goes beyond traditional "signposting," involving mental health triage, short-term low-level support, navigation of neurodiversity pathways, and liaison with schools and external agencies. The Link Worker also plays a key role in managing referrals for local youth projects and maintaining clear communication channels between GP practices, schools, parents, and statutory services.</p>

  <p><strong>The Role and Person</strong></p><p>You will be responsible for delivering outcome based psychological therapy that is uniquely tailored to meet the needs of service users living in the region. You will supervise a number of assistant psychologist supporting them in a manualised treatment approach, whilst working with the wider team on more complex cases. You will be offered supervision from a consultant Psychologist working within our service, they will encourage you to develop your strengths and skills with opportunities for professional development. </p><p>The service is community based and appointments are offered where possible in the service users locality as well as remotely via telephone and video.   <p>As a respiratory therapy assistant practitioner, you will deliver excellent patient care along a care pathway across organisationalboundaries. Your role will involve supporting your senior colleagues to deliver individualised treatment plans for patients with complexrespiratory conditions.You will work alongside your allied health professional colleagues to deliver our recently established symptom management programmein a 1-1 and group setting.   <p>We are looking for an enthusiastic, committed clinician who has experience working with complex cases within the area of community mental health with individuals who struggle with complex trauma, severe mental health illness and / or personality disorder.</p><p>Applications are invited for qualified clinical associate psychologist looking to extend their clinical and team working knowledge and experience.</p><p>The postholder will use a range of evidence-based psychological models and protocols when addressing individualised patient need and critically appraise these models and interventions (e.g. CBT, DBT, CFT) to inform treatment planning and appropriate choice of treatment protocols and understand how to interpret treatment choices with individuals, groups and other healthcare colleagues, when managing complex and chronic needs.</p><p>The role offers exciting opportunities for direct clinical interventions and multi-disciplinary team working, implementing evidence-based programmes of group and individual psychological therapies as well as providing consultation and supervision to other team members.   <p><strong>The Community Therapy Service team in Wallingford is looking for an enthusiastic clinician with an interest in Mental Health to join the team as a Senior Band 6 in a part time capacity of 30 hours per week.</strong></p> <p><strong>Could this be you?</strong></p> <p>We are looking for a senior clinician with a mental health background, who is enthusiastic to develop their clinical practice within innovative and supportive multidisciplinary teams, treating patients in their own homes.</p> <p>The Community Therapy Service aims to provide high quality integrated therapy services working as a key member of the community MDT . We work closely with other multidisciplinary team members, social services and voluntary organisations for adults with a wide range of physical and mental health conditions.</p>



  <h3>Main duties of the job</h3>
  <p>As a Mental Health Practitioner - Physiotherapist you will provide high quality mental health rehabilitation to patents referred to or within the Community Therapy Service (CTS). You will use your expertise to help support other CTS practitioners achieve rehabilitation goals. In addition to this you will be supporting our community response physical health pathway.</p>
<p>You will also work closely with the Community Mental Health Team (CMHT) to minimise duplication, maximise efficiency and facilitate a seamless pathway between the two services for the patient understanding the roles and responsibilities of the clinicians for establishing and delivering a care plan for the patient.</p>
