  <p>The Mental Health Support Team are currently recruiting for 2 Education Mental Health Practitioners (EMHP/EWP/CWP) to work with our Richmond schools.</p>



  <h3>Main duties of the job</h3>
  <p>Mental Health Support Teams in Schools (MHSTs) are an exciting development, located in schools and AFC offices. We provide a whole school approach to mental health; including individual and group work with students and parents, staff training and consultation and policy and ethos advice.</p><ul><li>The team is part of the broader Emotional Health Service (EHS) and links closely with AFCs other education and social care teams.</li> <li>Our service covers Richmond and Kingston boroughs. We have a strong sense of team and collaboration to support children and young peoples emotional wellbeing to achieve positive outcomes.</li> <li>The MHST is a dynamic service of Counselling &amp; Educational Psychologists, CBT Therapists, Dramatherapists, Dance &amp; Movement Psychotherapists, Music Therapists, Art Therapists, Systemic Practitioners, Counsellors, Education Wellbeing Practitioners (EWPs) and Child Wellbeing Practitioners (CWPs).</li> <li>The post holder will also be part of a team of clinicians who can provide access to clinical supervision, peer supervision, line management</li></ul>

Together we will build on the experience and knowledge you have already gained to enable you to safely assess and treat a wide range of complex podiatry conditions for patients with a variety of other clinical needs, including: acute or chronic, neuropathies, chronic diseases, vascular problems, wound management and nail surgery.</p>

We work closely with our local acute Trust to support their high-risk diabetic out-patient clinics and there will be opportunities to shadow and develop your skills in this area. We also manufacture our own insoles using state of the art CAD-CAM technology and pressure mapping technology as well as developing new pathways and services to improve patient experience and outcomes.</p> <p>Registered with the Health and Care Professions Council as a podiatrist (or on qualification), you will possess excellent communication and interpersonal skills with the ability to develop and maintain effective working relationships with GPs and other healthcare professionals. Good IT skills are essential as the role requires working across a range of clinical systems. </p> <p>We will provide a fully structured induction programme and weactively support CPD and personal development for all staff and we will provide opportunities to develop within the role.</p> <p>The Podiatry Service is delivered from a number of community clinics, GP practices as well as nursing homes and domiciliary settings across Wirral, therefore you must have access to your own transport for work purposes. </p> <p>Wirral has excellent transport links to North Wales, Liverpool, Manchester, as well as the historic city of Chester. </p>

  <p>This is a community-based post, located within the CAMHS in Social Care Team (CISC) here in Tower Hamlets. The team are based across the CAMHS Emanuel Miller Centre in Limehouse as well as the Local Authority Town Hall in Whitechapel. The CiSC team offers accessible, flexible, timely and responsive CAMHS input to children, young people and their families who are subject to Social Care plans (LAC, CP or CIN).</p> <p>It is a multidisciplinary team offering consultation to Social Worker and the wider children's workforce as well as assessment, care planning and clinical intervention to the children and families that they work with where there are mental health concerns. The post holder will require a high degree of professional autonomy and self-management and will be required to account for their professional practice through appraisal and audit.</p>

       Job Summary <p>The purpose of this post is to support the Clinical Lead and Inpatient Service managers with the overall capacity and flow on the general adult and older people acute wards across the Trust.</p> <p>The post holder will assist in in the operational management of beds within acute services and will work as a key player in the bed management service.</p> <p>The post holder will be required to attend service meetings to support capacity and flow, sourcing NHS placements and coordinating assessment and funding for such placements.</p> <p>The post holder will be the main source and support the Bed Managers in redirection of admissions in accordance with assessment of need through the identification of the appropriate care setting such as PICU, forensic services or alternatives to admission.</p> <p>The post holder will work closely with clinical multi-disciplinary team to ensure good communication between staff, patient and their relatives in regards to admission and discharge.</p> <p>The post holder will oversee and work with the Bed Managers to ensure accurate bed states with real time patient tracking expected to be maintained, as well as managing the bed capacity and provide availability for future admission planning.</p> <p>The post holder will oversee the data collection for monthly statistics, including OAPS and DTOCs and they will have a good degree of computer literacy in the use of NHS database systems.</p>

  <h3>Main duties of the job</h3>
  <p>ou will be a registered staff nurse and have excellent assessment, planning and evaluation skills to deliver clinical care to patients and support our Nursing Assistants.
You will coordinate and manage care ensuring that all calls are effectively assessed, triaged, allocated and recorded. You will have an overview of the service as a whole to ensure that patients and staff remain safe and care is appropriate and effective overnight. E.g. re-allocating work to cover unexpected staff absence.  At times you will be expected to work clinically and deliver direct patient care, though the majority of your role will be clinical triage via telephone. 
You must have excellent communication, organisational and IT skills as you will be coordinating care for a large number of complex patients.
You will provide clinical leadership, including line management of Band 5 Staff Nurses, and oversee line management of our Band 3 Nursing Assistants.
You must be able to think on your feet, problem solve and juggle conflicting priorities as well as motivate others in challenging times and utilise the resources available to you effectively.
The hours of work are 17:00pm-22:00pm over 7 days a week (including bank holidays).  The role is based from Hunslet Health Centre, though it is expected that you could travel across the city of Leeds during your shift to deliver patient care. 
It would be expected that you are available to work on a rota over 7 evenings per week, 365 days per year.
