Primary Care Mental Health Nurse (Advanced Practitioner)
The Mental Health Nurse will receive referrals in a number of ways: By direct booking into a clinic slot as advised by a clinician Via NHS Mail from clinicians at either practice. By mutual discussion within the team Suitable referrals can include patients with Anxiety and Depression, PTSD, Personality Disorder (maybe waiting for an appointment with a tertiary service). Undertake holistic assessments and co-produce a personalised plan with patients, identifying their support needs. Record all information on the practice clinical systems. These will be either face to face or as a telephone appointment. Medication reviews will be an important part of assessments and ongoing reviews and we hope to recruit someone who has knowledge of this and is an independent prescriber or can work towards this. Attend and feedback at locality and practice-based multidisciplinary team meetings as required. Work with members of the integrated care teams to support a joined-up approach to provision where needed. The Mental Health Nurse should be able to identify when there is a need for urgent action or for a step-up in care and know when to seek advice and support from the GPs and the wider team to discuss patient-related concerns, referring the patient back to other suitable health professionals or the core team/crisis team when required.
