What you will do:
- Act as care coordinator for a defined caseload of service users with severe and enduring mental health problems.
- Assess health needs, plan, implement, and review care packages in line with the Care Programme Approach (CPA).
- Administer medication and monitor physical and mental health side effects.
- Manage risks proactively through structured risk assessments and safety planning.
- Liaise closely with GPs, inpatient units, social services, and voluntary sector partners to ensure coordinated and comprehensive care.
