Community Mental Health Nurse - Lead Caseload & Rehabilitation

Rotherham Doncaster and South Humber NHS Foundation Trust is seeking a Community Psychiatric Nurse to work within our community mental health teams. You will provide comprehensive community-based care, lead caseloads, assess needs, plan and implement care with patients and carers, and collaborate with multidisciplinary colleagues to promote recovery and quality of life.

This role involves coordinating reviews, attending case conferences, and using SystemOne as the primary records system.
