Community Mental Health Nurse - Impactful Community Care

West London NHS Trust is seeking a Band 6 Community Psychiatric Nurse to join the MINT Team on a 6 month secondment, delivering community mental health support to adults in the London Borough of Hounslow.

You will manage a diverse caseload with adults experiencing mental health difficulties, in a multidisciplinary team. The role requires community experience, strong assessment and risk management skills, and a commitment to high quality, patient centred care.
