Advanced Practice Mental Health Nurse - Integrated Care

Allscreens Nationwide Ltd. is recruiting a Clinical Practitioner to deliver timely, person-centred care for adults with low to moderate mental health needs within a PCN setting. You will conduct assessments, develop shared care plans, and support patients with brief interventions and education.

You will refer to IAPT, CMHTs and crisis services as needed, working with GPs, ANPs, Social Prescribers, and OTs to plan joined-up care and to improve population health outcomes.
