personalised and coordinated health and social care to the 70000 patient

  <h3>Main duties of the job</h3>
  <p> Provide one-to-one coaching to
help patients identify goals and take steps towards improving their health and
wellbeing.</p><p> Support patients with
lifestyle change areas such as weight management, physical activity, healthy
eating, stress management, sleep, and smoking cessation.</p><p> Use behaviour change
approaches including motivational interviewing, active listening, and goal
setting to encourage patient confidence and self-management.</p><p> Deliver coaching remotely or
face-to-face across primary care sites.</p><p> Connect individuals with peer
support, self-management tools, and community activities to build confidence,
resilience, and social connection.</p>

  <p>Windsor Primary Care Network has been successfully
delivering at scale Care to the population of Windsor for 4 years. We have a
large team of both clinical and none clinical staff.</p><p>While each Health and Well-being Coach will become embedded
in up to 3 Practices, they frequently enjoy a mutually supportive joint
approach to work with other members of the 13 strong PCN Health and Wellbeing

<h2>Job summary</h2>
  <p><strong>Are you a newly qualified/soon to qualify Psychological professional looking for a new role in adult mental health? Or someone Psychological professional wanting to develop your systemic therapy skills?</strong></p>
<p>We are recruiting a part-time <strong>Clinical/Counselling Psychologist</strong>or <strong>Psychological Therapist</strong> with training in Systemic Psychotherapy or willingness to train in systemic therapy, to provide psychological assessments and interventions for adults with severe and/or complex mental health problems, their families and carers. This role is <strong>part-time working 30 hours per week.</strong></p>
<p>We also welcome applications from Clinical/Counselling Psychologists, or Psychological Therapists who are due to qualify soon.</p>
<p>Our enthusiastic, skilled and supportive team provides a range of therapies. You would contribute to the work of the team by joining the highly valued family therapy clinic and undertaking group and individual therapy with people seeking formulation-based CBT for developmental trauma and other complex difficulties.</p>



  <h3>Main duties of the job</h3>
  <p>You should be able to demonstrate your use of evidence-based practice in specialist adult mental health settings, including competence in cognitive-behavioural and third wave approaches and systemic interventions. If not already accredited, you will be willing to work towards accreditation with an accrediting body such as BABCP and/or AFT for which you will be fully supported.</p>
<p>As a member of Specialist Psychological Therapies you will work closely with colleagues in our adult mental health community and in-patient teams. You will also liaise with and support psychological therapists working at earlier steps in the Buckinghamshire Psychological Therapies Pathway. You will work with the Clinical Lead, Locality Leads, Team Manager and your team to ensure that the Buckinghamshire Psychological Therapies Pathway achieves its key objectives of delivering integrated, safe, accessible, responsive, effective and caring services to people in Buckinghamshire. You will ensure the service you provide is of a high quality and meets national guidance as well as meeting local and nationally agreed targets.</p>
<p>Please note if you are based outside of the UK you will need to hold HCPC registration in order to be considered for this role. The ability to travel independently between sites within the Trust is essential for this role.</p>

  <p>The CAP role is central to the team's psychological provision, with opportunities for group interventions, 1:1 therapy using a Structured Clinical Management model and other evidence based approaches, alongside joint assessment, formulation, intervention and consultation.</p> <p>There will be opportunities for leadership, research, service development and consultation to the system, which will involve liaising with professionals involved in the in care of the individual. The teams work closely as MDTs, with weekly clinical meetings where complex cases can be thought about and MDT formulations can be developed.   <p>The post holder will be involved in working within the CAMHS hospital team care pathway, assessing young people that present to an acute hospital and are experiencing a mental health crisis. Clinicians will be expected to undertake a comprehensive mental health needs assessment and be able to identify the forward care pathways for the young person and their carer(s).</p>
<p>We work across all localities including North Bristol, East &amp; Central Bristol, South Bristol, South Gloucestershire and North Somerset, and aim to build strong working relationships with all of these services. We cover a 7 day period working a number of shifts. These are 8-4pm, 12-8pm and 4pm until midnight and weekend working.</p>
<p>A proportion of the role will involve working in conjunction with colleagues in the multidisciplinary team to identify care and treatment options which enable service users to remain within their existing support/care pathways and maintain their community tenure, signposting to other services to meet their needs.</p>
<p>The post holder will maintain close working relationships with Community and specialist CAMHS services, Adult Community Mental Health Teams as well as the Acute Hospital Services. Day to day leadership on service delivery and development will be provided by the Team Manager, along with senior clinicians as will caseload and management supervision.</p>

Visit Health and Care Worker visa: Overview - GOV.UK</em></p> <p><strong>Clinical Psychologist, Eating Disorder Service</strong></p> <p>EDYs provide support and treatment for children and young people in Liverpool and Sefton with an eating disorder and/or ARFID, as well as their parents/carers and professionals involved in their care. This is a multidisciplinary team including Consultant Psychiatrist and Consultant Paediatricians, Clinical Psychology, Family Therapy, CBT and SFP trained staff, Nursing, Support Worker, Assistant Practitioner and Assistant Psychology.</p> <p>The successful applicant will provide a substantial contribution to the clinical work of the team and will hold full case management responsibility for their workload. Regular clinical supervision will be provided.</p> <p>The main base for this post will be at Alder Hey, but appointments across Liverpool and Sefton will be required.</p> <p>Experience working with children and young people with an eating disorder and/or ARFID will be beneficial. In addition to undertaking assessments and providing intervention and support to children, young people and their families, the post holder will also undertake physical monitoring under the guidance of our Consultant Paediatricians.</p>



  <h3>Main duties of the job</h3>
  <p>To provide highly specialised assessment and therapeutic interventions and to offer advice and consultation to other professionals across a range of agencies including: primary care, social care, and education.</p>
<p>The post holder will have clinical responsibility for complex therapeutic and risk management work with children and young people who have been assessed to require specialist support for eating disorders</p>
<p>The services comprise several multi-disciplinary teams including staff from varied backgrounds, including psychology, psychiatry, social work, nursing and psychotherapy. In addition, our team members are trained in a wealth of different therapeutic interventions and approaches, making the teams truly multi-disciplinary.</p>
<p>The post holder will provide both clinical and line manager supervision to junior members of the team as well as a leadership role within the service.</p>
