  <p>The Home Treatment Crisis Resolution Team is based at Bronllys Hospital, near Brecon. The team provides Acute Psychiatric Care to individuals at home rather than inpatient care. The service operates between 9:00am and 9:00pm, 7 days a week. The service is provided to the populations of Breconshire, Radnorshire and Ystradgynlais. The team works closely with the Community Mental Health Teams based in the catchment area and inpatient ward.</p> <p>The role includes assessing the suitability of home treatment for individuals presenting with acute mental illness or severe distress, which can be safely managed in the person's home.   <p>To work as a Team member in providing a Crisis Resolution Home Treatment Service (CR/HT) in Powys. The CR/HT service will provide a service for adults who are experiencing a Mental Health crisis and are at risk of admission to Hospital. Intensive Home Treatment will be provided in collaboration with the service user and in the least restrictive setting, 7 days a week, and 365 days a year as per rota. CR/HT will also facilitate the early discharge of those admitted to inpatient units, through intensive but time limited intervention in the community.</p> <p>The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.</p>

Women can be admitted to New Dawn if they have a diagnosis of personality disorder, exhibit self-harming behaviour and are able to accept the need for help. Service users can be informal or detained under the Mental Health Act.
Upping Ward is our 15 bed intensive Support Service for women with a diagnosis of personality disorder and complex mental health needs. The service pathway provides a robust programme of care for women with challenging high risk behaviour who experience difficulties with engagement with therapeutic programmes. We aim to meet the needs of these women, who may have had previous inappropriate placements, in a less restrictive setting.</p>

  <p>Cygnet Hospital Beckton is a is a gender specific hospital for women with complex mental health needs.
Situated in the East End of London, the hospital has four different specialist wards including PICU, acute admissions and personality disorder services.
We accept emergency and planned, pre-assessed admissions. Our aim is to support individual service user recovery and integration back into the community, as quickly as possible. We have a multi-disciplinary team of specialists and offer a very broad range of therapies.</p>

<h2>Job summary</h2>
  <p>The post holder will be working within a busy team alongside a team ofregistered mental health nurses and be supported by the Team Manager,Senior Clinical Lead/CBT Practitioner, and team administrators.</p> <p>Post holders will have a high level of autonomy working across designated GP practices, managing their own clinics and providing mental healthassessment and evidenced based treatment for people aged over 18. You will provide a service to people who exhibit complex common mental health needs and stable, predictable and enduring serious mental health needs. You will be expected to develop and manage all aspects of their service, such as waiting lists according to local need, working within the parameters of the service operational policy.</p> <p>A key aspect of this role is GP consultation, liaison, education and training. You will be expected to attend GP practice meetings at least twice a year to discuss the service and actively seek feedback and report any issues to the team leader as well as areas for improvement and any innovative practice for sharing and disseminating.
