  <p>We are looking for a highly motivated, experienced and innovative Band 6 mental Health Practitioner to join our CMHT. Winchester CMHT is a dynamic, nurturing, and supportive team with excellent peer support and regular supervision. You will manage a reasonable-sized caseload of service users, covering shared care and carrying out assessments. You will be involved in training, mentoring and supervising new and unregistered staff as well as providing excellent front-line clinical care.</p><p>Winchester CMHT provides compassionate, kind and recovery-focused care to our patients, carers and their families. You will need to be able to work autonomously in the community and use your own initiative. The successful candidates will have sound clinical skills, administration and IT skills including be able to type. You will also need excellent assessment and communication skills. There will be a range of shadowing/training opportunities to introduce you to the care coordination role and you will need to work alongside other agencies to promote holistic recovery-focused care.</p>



  <h3>Main duties of the job</h3>
  <p>To act as care coordinator to an agreed caseload of service users with severe and enduring mental illness.</p><p>To supervise the work of a small team of practitioners within the wider Community Treatment Team.</p><p>To undertake ongoing assessment, treatment and evaluation for service users known to the Service.</p><p>To deputize for the Team Leader as required.</p><p>To work in a flexible hour's service (this includes being on a staff rota providing services outside of office hours and at weekends and bank holidays).</p><p>To coordinate the care of an agreed caseload of service users with severe and enduring mental illness.</p><p>To provide clinical supervision and caseload management to junior colleagues</p><p>To formulate care plans in line with identified need.</p><p>To provide therapeutic interventions in line with the agreed care plan and risk assessment</p><p>To assess risk, develop risk management plans, review, and adjust in line with the service user's needs.</p>

  <h3>Main duties of the job</h3>

<p>Our AMHT provide an assessment and treatment function and the staff working within the team work on a rota covering shifts from 07.00 to 21.00 (7 days/week). The successful candidate will be required to work mainly 9-5 Monday to Friday however there is an expectation that there is flexibility to cover the out of hours shifts when required.</p>

  <p>At NELFT, we are proud to be the largest provider of Mental Health Support Teams (MHSTs) in England. Our teams deliver evidence-based interventions in schools and colleges for children and young people aged 4-18, while also supporting Whole School Approaches to improve wellbeing across education settings. Working closely with school-based services and linking with CAMHS where needed, MHSTs play a vital role in strengthening mental health support for young people.</p>   <p>We are seeking an experienced and motivated Senior Mental Health Practitioner (SMHP) to join our Mid Essex MHSTs. You will deliver CBT interventions, lead Whole School Approach activities, and support Education Mental Health Practitioners through supervision and guidance. The role also includes developing school networks, delivering staff training, supporting group programmes, and co-producing mental health resources.</p>   <p>Applicants should be CYPMHS clinicians undertaking or having completed CYP-PT CBT or SFP training, or registered professionals with a postgraduate diploma in evidence-based psychological therapies. Strong CBT experience is essential.</p>   <p>You will work closely with schools to embed MHST support, develop materials for interventions, and promote positive mental health across education communities.   <p>ou will be a registered staff nurse and have excellent assessment, planning and evaluation skills to deliver clinical care to patients and support our Nursing Assistants.
You will coordinate and manage care ensuring that all calls are effectively assessed, triaged, allocated and recorded. You will have an overview of the service as a whole to ensure that patients and staff remain safe and care is appropriate and effective overnight. E.g. re-allocating work to cover unexpected staff absence.  At times you will be expected to work clinically and deliver direct patient care, though the majority of your role will be clinical triage via telephone. 
You must have excellent communication, organisational and IT skills as you will be coordinating care for a large number of complex patients.
You will provide clinical leadership, including line management of Band 5 Staff Nurses, and oversee line management of our Band 3 Nursing Assistants.
You must be able to think on your feet, problem solve and juggle conflicting priorities as well as motivate others in challenging times and utilise the resources available to you effectively.
The hours of work are 17:00pm-22:00pm over 7 days a week (including bank holidays).  The role is based from Hunslet Health Centre, though it is expected that you could travel across the city of Leeds during your shift to deliver patient care.

  <p>Are you a qualified Education Mental Health Practitioner? Have you (or are you about to) complete your EMHP training? Then Leeds is the place for you!</p><p>This is an exciting opportunity to work within the MindMate Support Team (MHST) based in the City of Leeds. We currently have various EMHP posts available across the city due to lots of internal promotions as our service grows.</p><p>You will be employed as a Band 5 Education Mental Health Practitioner, working with colleagues across the city of Leeds, with education settings and health professionals to offer Mental Health and wellbeing support to young people up to the age of 18 and their families.</p><p>We will require successful applicants to be registered (or going through the process) with either the BPS or BABCP as you will be working within a NHS commissioned service.</p><p>You will work within LCH NHS Trust and provide a range of assessment and other interventions with individuals and wider systems under close clinical supervision from our experienced staff.</p><p>Regular case management and clinical supervision on an individual basis and peer group supervision will be offered to ensure excellent clinical standards and registration is maintained.</p><p>The MindMate Support Team holds a strong culture around wellbeing and work life balance and supports a hybrid model of working.   <h3>Main duties of the job</h3>
  <p>The post-holder will have completed or be nearing the end of the HEE postgraduate diploma in Education mental health practice. They will be able to competently deliver a range of 1:1 and group Low Intensity CBT based interventions for children, young people, and parents/carers.</p><p>Main duties of the job involve using your skills and knowledge to deliver a range of whole school work such as assemblies, workshops and education staff training. Alongside 1:1 and group Low Intensity CBT based interventions for young people, and parents/carers.</p><p>Delivering a 'whole setting approach' within educational settings developing excellent professional working relationships along the way. You will work within a strongly supportive mini team including a fellow EMHP and a practitioner.
