  <p>The Chronic Pain Self Management Team is a well established and experienced multi disciplinary team offering highly specialist assessment and intervention for people living with complex chronic pain presentations. We aim to support people referred to our service to develop a range of self management skills to allow them to improve their overall quality of life despite pain. Interventions are focussed on improving understanding of chronic pain, increasing function and engagement in valued activities, managing pain related distress and reducing inappropriate health case. </p> <p>This post would suit a Physiotherapist who is looking to increase their skills in chronic pain / long term condition management using a biopsychosocial framework. Support will be provided to assist the post holder to -o Increase their knowledge of complex chronic pain presentationso Deliver self management interventions in groups and with individualso Develop knowledge and skills in delivering approaches based on CBT/ ACT / mindfulness models</p> <p>Specialist supervision and support will be provided by senior physiotherapists in the team. </p>



  <h3>Main duties of the job</h3>
  <p>To provide a high standard of service to patients attending the Chronic Pain Service working as an autonomous practitioner, with advice and support from Band 7 and 8a Physiotherapists, the service Clinical Lead and other team members. The post holder will work with patients presenting with complex chronic pain presentations, offering specialised clinical assessment, treatment planning and appropriate discharge from the service, adapting practice to suit the needs of the patient. </p>

  <h3>Main duties of the job</h3>
  <p>To assume responsibility for management of a defined caseload and exercise the highest standards of professional responsibility for the assessment, treatment and discharge of children, young people and their families/carers using specialist knowledge and evaluation of care programmes in collaboration with children, young people and their families/carers, liaising with and referring to relevant agencies.</p> <p>To participate in a duty rota including receipt and screening of referrals, obtaining further information if needed and responding promptly to emergency requests for assessments or contact, signposting, and advice.</p> <p>To undertake assessment of children, young people including risk assessment, gathering appropriate information their family/ carers and other relevant agencies</p> <p>To develop a formulation and plan for intervention with the child, young person and parents/carers. Communicate this to the referrer/other relevant professionals</p> <p>To offer therapeutic, evidence-based practiced, interventions in line with NICE guidelines.</p> <p>To implement, co-ordinate and regularly review further assessment and a range of therapeutic interventions, including risk management,</p> <p>To routinely monitor effectiveness of clinical work by use of Routine Outcome Measures (ROM)</p> <p>Ensure that the perspective of children, young people and families is incorporated into all aspects of assessment and treatment</p> <p>The role involves travel to other locations within MFT and within the local community.</p>

<h2>Job summary</h2>
  <p>We are looking for 12 month maternity cover to the Band 7 Senior Mental Health Practitioner role within the North Bristol CAMHS team.</p> <p>We are a service working withchildren and young people with complex mental health needs. We use the CAPA model of assessment and treatment, which would involve working with a variety of clinical presentations and needs, including: suicidal thoughts, low mood, anxiety, OCD and emerging psychosis, often alongside neurodiversity. We also have a lot of referrals for young people with disordered eating and diagnosed Eating Disorders, and there are opportunities to work within this clinic if this is something you are interested in.</p> <p>We are looking for a practitioner with a core professional registration, including registered nurses, social workers, psychologists, occupational therapists, family therapists, art therapists and CBT therapists.</p> <p>North Bristol is an area of significant disparities in health, wealth and access. <li>Utilising your own clinical supervision, reflective practice and caseload management to work in an evidence-based manner and in a way that is sustainable for your own wellbeing.</li>
