  <p>Sefton Enhanced Support Team is looking for an experienced Mental Health Practitioner to join our service.</p> <p>The Enhanced Support Team works with children and young people with complex needs who present with high risk, high harm behaviours and high vulnerability. They are likely to have difficulties in multiple domains which are persistent, severe and are framed by family and social contexts (family disruption, loss, ACEs). This may include for example, children and young people who are involved in youth justice services, not in education employment or training, who engage in substance misuse, have multiple adverse childhood experiences, who are neurodiverse or have speech language and communication difficulties etc. These children and young people are likely to have been referred to multiple services, but there are significant barriers to engagement</p> <p>Sefton Enhanced Support Team has links into Community CAMHS however is integrated with Children's Services including Social Care and Youth Justice.</p> <p>The successful applicant will provide a substantial contribution to the clinical work of the team and will hold case management responsibility for their workload. Provision of supervision to lower banded team members, students and other internal professionals as required is expected.</p> <p>We are particularly seeking candidates who have experience of working within complex systems, providing case consultation and working with families indirectly via wider professional involvement.</p>

Mental Health Practitioner - Enhanced Support Team (Sefton)

<h2>Job summary</h2>
  <p>An opportunity has arisen for a band 4 Generic Therapy Technical Instructor to work within Ty Croes Atti Discharge to Recover and Assess Unit (D2RA). Ty Croes Atti aims to provide a 24 hour approach to rehab, promoting independence and assessing a service user's care and rehab needs with the ultimate goal of returning home.</p>
<p>We are looking for a motivated, hard-working, proactive individual who will be able to work alongside Physiotherapists, Occupational Therapists, Social Workers, other TIs and Support Workers. The role sits within the Flintshire Unscheduled Care Team where we value the importance of continuing professional development and Peer support. The successful candidate will be supported with supervision, appraisals and identifying learning opportunities and training.</p>



  <h3>Main duties of the job</h3>
  <ul>
<li>Gathering of baseline information to assist the Therapy Assessments.</li>
<li>Carry out therapeutic interventions following initial assessments carried out by the Physiotherapist and Occupational Therapist.</li>
<li>Interventions will include progression of transfers and mobility, delivery of exercise programmes including exercise classes and practicing activities of daily living such as washing and dressing and meal preparation.</li>
<li>As a Band 4 TI you will also work independently to promote wellbeing, safety and independence within essential activities of daily living, by providing instruction, advice and treatment to the service-user and/or their carers.</li>
</ul>
<p>The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.</p>

  <p>You will be responsible for reviewing nutritional care plans for adult renal patients at Manchester Royal Infirmary. The role will involve communicating with members of the wider multidisciplinary team including doctors, nurses, allied health professionals (physiotherapists, occupational therapists and speech and language therapists), pharmacists and the catering department.</p> <p>There will be opportunities to contribute to patient education and staff training sessions. You will have a key role in delivering health promotion campaigns such as Dietitians Week, AHP (allied health professional) week, and Nutrition and Hydration week. You will support the dietitians and have responsibility for some administrative tasks to ensure the smooth and effective running of the department.</p> <p>Although previous experience in health and care is not required it is favourable. We work closely with our local acute Trust to support their high-risk diabetic out-patient clinics and there will be opportunities to shadow and develop your skills in this area. We also manufacture our own insoles using state of the art CAD-CAM technology and pressure mapping technology as well as developing new pathways and services to improve patient experience and outcomes.</p> <p>Registered with the Health and Care Professions Council as a podiatrist (or on qualification), you will possess excellent communication and interpersonal skills with the ability to develop and maintain effective working relationships with GPs and other healthcare professionals. Good IT skills are essential as the role requires working across a range of clinical systems. </p> <p>We will provide a fully structured induction programme and weactively support CPD and personal development for all staff and we will provide opportunities to develop within the role.</p> <p>The Podiatry Service is delivered from a number of community clinics, GP practices as well as nursing homes and domiciliary settings across Wirral, therefore you must have access to your own transport for work purposes. </p> <p>Wirral has excellent transport links to North Wales, Liverpool, Manchester, as well as the historic city of Chester. </p>

  <p>We are seeking a Highly Specialist Speech and Language Therapist who is passionate about Neurorehabilitation (NR) for a 12 month fixed term contract or secondment. Applicants with relevant clinical experience seeking opportunity of a more senior role may also be considered. The post holder will gain excellent clinical expertise and leadership skills across the neuro rehab pathway working with our specialist teams and support patients accessing the Level 1 units in Salford, and Level 2 unit in Rochdale (Floyd Unit). Site base will be dependent on skills and experience of successful applicant.</p> <p>Applicants must have established skills in management of dysphagia, acquired communication disorders, and experience of working with patients with tracheostomy. Complex neuro presentations include patients on the 'Prolonged Disorder of Consciousness' (PDOC) spectrum and 'Cognitive Communication Disorder' (CCD). </p> <p>You will be joining a dynamic and friendly MDT and will also be part of the larger Neuro Rehab SLT service. We are a team that pursues innovation, and support CPD and research opportunities. Examples of previous pilots and collaborations include Phagenyx, Music therapy, collaboration with Ace Centre and the ongoing Facial Rehab Pathway Project work with the 'Greater Manchester Neurorehabilitation and Integrated Stroke Delivery Network' (GMNISDN). </p> <p>Informal visits and enquiries are welcome. </p>

  <p>To join our Neurorehabilitation service on a Fixed Term/secondment basis and support the limited band 7 rotation covering our Level 1 units at Salford (C2 and L1) and Level 2 unit (Floyd Unit) in Rochdale. Site base will be dependent on skills and experience of successful applicant.</p> <p>-To provide highly specialist evidenced based input to the provision and development of Speech and Language Therapy in NR.</p> <p>- To lead on and coordinate management of complex communication disorders within this caseload, including patients with; tracheostomy, 'prolonged disorders of consciousness' (PDOC), 'locked in syndrome' and 'cognitive communication disorder'. </p> <p>-To lead on and coordinate management of complex dysphagia within this caseload, including patients with tracheostomy.</p> <p>- ependent on existing skills you may have opportunity to join the Salford VFS and FEES rota and will be required to support the mobile FEES service for INRU within MCCN.</p> <p>-You will support the Senior management team with service development projects for Neurorehabilitation and in line with the Greater Manchester transformation project. Networking with other Neurorehabilitation services and the Greater Manchester Network will be part of your role.</p> <p>There may be opportunity to join Salford VFS and FEES rota, and support of the mobile FEES service for the Level 2 units within MCCN, dependent on existing skill.</p>
