Well-being Coach, to work within our Primary Care Network multidisciplinary
healthcare team.</p>



  <h3>Main duties of the job</h3>
  <p> Provide one-to-one coaching to
help patients identify goals and take steps towards improving their health and
wellbeing.</p><p> Support patients with
lifestyle change areas such as weight management, physical activity, healthy
eating, stress management, sleep, and smoking cessation.</p><p> Use behaviour change
approaches including motivational interviewing, active listening, and goal
setting to encourage patient confidence and self-management.</p><p> Deliver coaching remotely or
face-to-face across primary care sites.</p><p> Connect individuals with peer
support, self-management tools, and community activities to build confidence,
resilience, and social connection.</p>

delivering at scale Care to the population of Windsor for 4 years. We have a
large team of both clinical and none clinical staff.</p><p>While each Health and Well-being Coach will become embedded

approach to work with other members of the 13 strong PCN Health and Wellbeing

  <p>An exciting and challenging opportunity has arisen for a permanent full-time band 6 Occupational Therapist within the Integrated Independence Team (Rapid Response Team).</p> <p>We are looking for an innovative, enthusiastic and experienced Occupational Therapist to be part of the Emergency department and community rapid response service within the Integrated Independence Team. This will be offered as permanent and full-time for an existing band 6 therapist or experienced band 5 therapist.</p> <p>The Integrated Independence Team is an integrated health and social care service provided jointly by Homerton University Hospital NHS Foundation Trust and London Borough of Hackney. The team places the user at the heart of the care planning process, and aims to promote independence, deliver outcome focused care, and provide a seamless journey through the care system.</p> <p>The team provides rapid response, intermediate care therapy and reablement services and consists of Occupational Therapists, Physiotherapists, Psychotherapists, Nurses, Speech and Language Therapists, Consultant Geriatricians, Social Workers, OT Technician, Care Support Coordinators, Care Support Workers, Therapy Assistants and Administration staff. The team will use integrated support planning to address health and social care needs across the pathway in a responsive, dynamic and flexible way.</p> <p>Working Hours: 37.5 hours per week (part time, flexible working or compressed hours will be considered).</p>



  <h3>Main duties of the job</h3>
  <p>We are looking for a Band 6 Occupational Therapist to provide a rapid response, senior Occupational Therapy assessment and treatment of complex patients within Emergency department, short stay unit and patients referred from the community in order to prevent admission to hospital and promote early discharge.</p> <p>In doing this you will work as part of a strong multidisciplinary team, within an interdisciplinary working model, to facilitate hospital discharge and prevent acute admission.</p> <p>The ideal candidate will have strong experience as an Occupational Therapist in an Acute Therapy or Rapid Response setting. You should have excellent communication skills, a proactive approach, prioritisation and problem-solving skills.</p> <p>You will be encouraged to develop your clinical, teaching and management skills through opportunities within the team and in the wider organisation. You will have support from a Clinical Specialist Lead and experienced staff across the team. Peer supervision and in service training are carried out on a regular basis and you will have an annual appraisal.</p> <p>There is excellent support for personal development and opportunities to develop the service in line with best practice. <p>You'll be joining an established, supportive Multidisciplinary Team providing rehabilitation to people with neurological conditions in their own homes. We work across the Community Neuro Rehabilitation Service and the Early Supported Discharge for Stroke service, helping patients regain independence and achieve the goals that matter most to them.</p>
<p>The Therapy Assistant role is pivotal, and you will work closely together with all of the other team members to support patients' rehabilitation, manage their neurological condition and achieve their goals.</p>
<p>As part of the role, you'll visit patients in their homes and in community settings, so access to your own vehicle is essential. After a thorough induction, you'll deliver rehabilitation and education sessions both independently and alongside other team members. <p>Due to significant business growth PAM Physio Solutions are looking to employ the skills of an enthusiastic and experienced occupational health occupational therapist to provide specialised workplace needs assessments and provide the opportunity to enhance their skills working for a large occupational health provider. They will be based and cover assessments in the South region as part of a nationwide team</p><p>We are looking for an&nbsp;<strong>Occupational Therapist</strong>&nbsp;who is passionate about the assessment of individuals with acute, chronic and complex health presentations to support their return to work or keep them in work. As part of our PPS team, you will be involved in delivering Specialist workplace needs assessments, Rehabilitation Services, Ergonomic Assessments, Case Management and delivering onsite training programmes.</p><p>The role will involve traveling to different client sites/ PAM clinics with some remote/ home-based work.</p><p>A full induction will be provided along with training and development through PAM’s dedicated PAM Academy, to support CPD and career aspirations</p><p>Travel time will be factored into the working day (depending on distance).&nbsp;</p><p><strong>Key Accountabilities and Responsibilities</strong></p><p>Working as an autonomous clinical practitioner, being professional and responsible for all aspects of own workload.</p><p>To deliver specialist workplace needs assessments and relevant services covering a range of health conditions at client sites/ PAM Clinics or remotely. Delivering a thorough and detailed clinical assessment, using appropriate clinical techniques, tools and equipment. Using clinical reasoning to provide relevant solutions and recommendations.</p><p>Delivering evidence-based intervention to all patients, in line with RCOT and NICE guidelines, and in accordance with the PAM PPS clinical pathways.</p><p>Formulating treatment plans / goals, assisting and enabling patients to achieve their goals and targets.</p><p>Providing appropriate advice based on the patient’s medical, social and cultural circumstances.</p><p>Maintaining accurate, comprehensive, contemporaneous patient records in line with clinical professional standards of practice.
