Primary Care Mental Health Practitioner - Community Impact

Northpoint Wellbeing Limited in Leeds is recruiting a Mental Health Practitioner to work across GP surgeries as part of the Leeds Mental Wellbeing Service. The role focuses on practical, compassionate support rooted in the community, providing assessment, triage, care navigation and short-term guidance to help people take steps towards wellbeing.

The post is full-time (35 hours per week) with Agenda for Change Band 5 pay (£32,073-£39,043).
