Part-Time Community Mental Health Nurse - Care Coordinator

Dorset HealthCare University NHS Foundation Trust is recruiting a Band 6 Community Mental Health Practitioner for the Shaftesbury Older Persons CMHT. The role is part-time (28 hours per week, Monday to Friday, 9:00am-5:00pm) and involves care coordination for individuals with complex mental health needs in the community.

You will undertake assessments, care planning, risk management, and multi-agency liaison to ensure safe, person-centred delivery.
