As part of our outreach team, you will:
- Provide personalised, community-based support to individuals with complex mental health needs, promoting independence within their own homes.
- Work closely with clients to implement and review tailored care plans, ensuring their safety and well-being.
- Collaborate with multidisciplinary teams, including clinical professionals, to deliver trauma-informed and recovery-focused care.
- Maintain accurate records and report any changes in client condition or concerns promptly.
- Participate in ongoing training to develop skills in mental health support and person-centred approaches.
- Essential qualifications include strong communication and interpersonal skills, empathy, reliability, and a willingness to learn and work flexibly within a community setting.
- Previous experience in social care or mental health support is desirable but not essential.
