Walsall Healthcare NHS Trust is recruiting a Mental Health Nurse to work across primary and secondary care, coordinating pathways for medically optimised patients and supporting discharge planning to improve bed utilisation and patient flow.
The post holder will work autonomously and within a multidisciplinary team to promote patient-centered care, assess and implement care plans, and drive quality improvements across Intermediate Care Service pathways in the community and hospital settings.
