Hybrid Mental Health Practitioner - Leaving Care

Manchester Mind is recruiting a Mental Health Practitioner (Leaving Care) to support young people aged 16-25 as they transition from residential care or custody. The role offers 35 hours per week with a salary of £32,271 per year and base at Victoria House.

You will work across community and home-based settings, reporting to the CYP and Counselling Services Manager. The role emphasises building trusting relationships, providing short- and long-term support, and engaging young people through
