Community Stroke Recovery Support Worker

Nottinghamshire Healthcare NHS Foundation Trust is seeking a part-time Stroke Community Rehabilitation Support Worker to join our Integrated Community Stroke Service in Nottinghamshire, delivering care in various community settings.

You will work within a multidisciplinary team to assess patients, implement therapy plans, and support independence after stroke, collaborating with OTs, PTs, SLT, psychologists and rehab staff.
