Community Psychiatric Nurse - City South LMHT (Band 5)

Nottinghamshire Healthcare NHS Foundation Trust in Nottingham is looking for a Band 5 Community Psychiatric Nurse to join the City South Local Mental Health Team. You will work in a diverse team, support patients to recover and remain well in their own homes, manage a caseload, and act as a Key Worker.

You will plan, facilitate and review patient-centred care with families, carers, and external agencies, contributing to MDT discussions and staff development.
