Community Mental Health Nurse: Assessment & Care Planning

Birmingham and Solihull Mental Health NHS Foundation Trust is seeking a registered nurse to manage the mental health needs of individuals referred to the team and to lead in developing and implementing therapeutic interventions.

You will work with a multi-disciplinary team including Occupational Therapist, Community Psychiatric Nurses and Support Workers, reporting to the Team Manager and providing duty cover while assessing and planning care and addressing clinical risk.
