What you will do
- Operate as an autonomous practitioner, conducting specialist nursing assessments in collaboration with service users and their families, and presenting back to the wider multidisciplinary team.
- Partner with service users and their families to develop treatment plans that ensure safety, promote well-being, and encourage independence within the community.
- Deliver evidence-based interventions in line with NICE guidelines to facilitate recovery and prevent relapse in various settings.
- Regularly review care outcomes, manage clinical outcome measures, monitor progress, and adjust treatment options as needed.
- Provide professional consultation to multi-agency meetings and forums.
- Contribute to the duty provision within the team, ensuring comprehensive care and support.
Responsibilities include
- Drawing on experience as a Community Mental Health Nurse to manage caseloads efficiently.
- Employing psychosocial interventions to enhance service user participation in their care.
- Demonstrating expertise in conducting assessments for new patients.
