We provide training, clinical supervision and access to a peer support network. </p><p>This role is crucial in supporting the Registered Nurses on an evening to enable patients to remain at home and avoid hospital admission.</p>



  <h3>Main duties of the job</h3>
  <p>You will support the Registered Nurse, responding to intervention call outs between the hours of 5pm and 10pm. You may also work alongside a colleague and visit patients to deliver planned care.</p><p>You must have excellent communication, organisational and IT skills.

It would be expected that you are available to work on a rota over 7 evenings per week, 365 days per year. Your hours of work will be 17:00pm - 22:00pm.</p><p>The role is based from Hunslet Health Centre, though it is expected that you travel across the city of Leeds during your shift, to deliver patient care.</p><p>The contract is for a minimum of 10 hours (2 evenings) up to a maximum of 30 hours (6 evenings). <h2>Job summary</h2>
  <p>Social Work Support Worker required to join a busy and diverse mental health social work team in the north of the county.</p> <p>Our team primarily support Older People who have previously or are currently experiencing mental health needs and require additional care and support, extending that support and guidance to carers and family, as a holistic approach to wellbeing.</p> <p>This role involves working directly with service users and their families, lone working or in partnership with other professionals and organisations, and requires a dynamic, positive personality to help achieve the best outcomes for our service users and team.You will have a good basic level of education and be able to travel independently around the county.</p>



  <h3>Main duties of the job</h3>

<li>To work with eligible users of Older Peoples Social Services to provide resilience and independence focused interventions. This will be achieved through ongoing support, practical assistance and problem resolution under the supervision of Social care.</li>
<li>To assess need and eligibility of service users supported by the team.</li>
<li>To contribute to the regular reviews of service users supported by the team.</li>
<li>To ensure these reviews in conjunction with MDT colleagues support the transforming lives and care act agenda.</li>
<li>To deliver person centred care and support focussing on collaborative relationships. This will include facilitating and empowering the person to develop personal and social networks that are meaningful to them. To aid and facilitate what the person defines as 'their good life'.</li>

Our department is equipped with state-of-the-art technology, including Elekta Linear Accelerators and a Halcyon system with Ethos software--enabling the delivery of advanced adaptive radiotherapy.  We use CRAD SGRT throughout the department. We also operate a busy and well-established brachytherapy service, treating a range of prostate, gynaecological, and skin cancers. Imaging is supported by a Toshiba wide bore CT scanner.</p>
<p>This competency-based role offers flexibility and scope for development. You will have the opportunity to rotate through all key areas of the department--including CT, pre-treatment checking, treatment, and brachytherapy--or, if preferred, specialise in an area aligned with your experience and interests.</p>

The role will include participation in shift patterns and extended hours to meet service capacity and patient demand.
