  <p></p><p>Windsor Primary Care Network, is a group of 6 GP
Practices working together with a range of local providers to offer more
personalised and coordinated health and social care to the 70000 patient
population of Windsor in Berkshire.</p><p>

</p><p>We are looking to recruit to the post of Health and
Well-being Coach, to work within our Primary Care Network multidisciplinary
healthcare team.</p>



  <h3>Main duties of the job</h3>
  <p></p><p> Provide one-to-one coaching to
help patients identify goals and take steps towards improving their health and
wellbeing.</p><p> Support patients with
lifestyle change areas such as weight management, physical activity, healthy
eating, stress management, sleep, and smoking cessation.</p><p> Use behaviour change
approaches including motivational interviewing, active listening, and goal
setting to encourage patient confidence and self-management.</p><p> Deliver coaching remotely or
face-to-face across primary care sites.</p><p> Connect individuals with peer
support, self-management tools, and community activities to build confidence,
resilience, and social connection.</p>



  <h3>About us</h3>
  <p></p><p>Windsor Primary Care Network has been successfully
delivering at scale Care to the population of Windsor for 4 years. We have a
large team of both clinical and none clinical staff.</p><p>While each Health and Well-being Coach will become embedded
in up to 3 Practices, they frequently enjoy a mutually supportive joint
approach to work with other members of the 13 strong PCN Health and Wellbeing

pro-active steps to improve the way they manage their physical and mental
health conditions, based on what matters to them. They support people to
develop their knowledge, skills, confidence in managing their health and care,
to improve their health outcomes and quality of life and support them making
changes in health related behaviours. Health coaches do this by coaching and
motivating people through multiple sessions to identify their needs and to set
goals for themselves, and through providing access to interventions such as
self-management education and peer support.</p><p>Health and Wellbeing Coaches manage and prioritise a caseload,
according to the needs, priorities and support required by individuals in the
caseload. They identify when a persons needs are beyond the scope of the health
and wellbeing coach role and refer them back to other health professionals or
organisations. Health and Wellbeing Coaches may work with people by phone, by
video conference, or face- to-face.</p><p>The successful candidate will be kind, reflective and
self-aware and will enjoy working with a wide range of people. They will have
good communication and negotiation skills and a firm belief that people have
untapped resources within them, that can be unleashed by providing high
quality, non-judgemental support. They will be proactive with a flexible
attitude, keen to work as part of a team and committed to developing themselves
in this skilled role and supporting colleagues to do the same.</p><p>They work closely with GPs and practice teams to manage a
caseload of patients, acting as a central point of contact to ensure
appropriate support is made available to them and their carers; supporting them
to understand and manage their condition and ensuring their changing needs are
addressed.</p><p>This is achieved by bringing together all the information
about a persons identified care and support needs and exploring options to meet
these within a single personalised care and support plan, based on what matters
to the person.</p><p>Health and Well-being Coaches review patients needs and help
them access the services and support they require to understand and manage
their own health and wellbeing, referring to social prescribing link workers,
health and wellbeing coaches, and other professionals where appropriate.</p><p>The successful candidate will be based in a local cluster of
General Practices as part of the Primary Care Network (PCN). They will be
caring, dedicated, reliable and person-focussed and enjoy working with a wide
range of people. They will have good written and verbal communication skills
and strong organisational and time management skills. They will be highly
