  <p>The Genomic Associate will be based in the Clinical Genetics Service at Northwick Park Hospital and will work alongside clinical geneticists, genetic counsellors, laboratory staff and other healthcare professionals to provide enhanced contact with patients at the point of referral, and after genetic testing and counselling, to facilitate the collection of information and obtain consent, where needed. The genomic Associate will also help to facilitate access to the service, particularly amongst marginalised communities and families who are difficult to engage. The post holder will be responsible for requesting test reports, relevant tissue/samples and testing by liaising with laboratories, families, and clinical teams, as well as the tracking of such requests They will keep clinicians and patients updated on progress, highlighting any long waiters.</p> <p>Under supervision, you will support the team with keeping spreadsheets, databases and portals updated for gene carriers, as well as undertaking regular audit to ensure patients are accepted onto screening programs. They will be responsible for arranging breast screening for those at high risk and keeping spreadsheets/records updated.</p> <p>the post holder will support the team with cancer triage by obtaining family history information, entering of data for online cancer risk assessments and generating a risk calculation using that program (e.g. CanRisk) for interpretation and use by genetic counsellors and clinical geneticists.</p>

<h2>Job summary</h2>
  <p>These are part time posts working 22.5 hours per week. However, we
would consider less hours for the successful candidate but would have to be a
minimum of 15 hours. The salary quoted is pro rata for part time hours.</p><p>We are looking for two motivated individuals with lived experience to support our current quality improvement programme on our older adult mental health inpatient wards. The post will be funded via the Royal College of Psychiatry`s Culture of Care Programme.</p><p>This programme aims to improve the culture of inpatient mental health, learning disability and autism for patients and staff so that they are safe, therapeutic and equitable places to be cared for, and fulfilling places to work.</p><p>Integrated Lived Experience takes the form of true co-production, the role focusses on creating a supportive and compassionate atmosphere for both staff and patients. There is an emphasis on developing change ideas in line with the programme alongside the wider MDT and core project team.</p><p>We can offer flexible working and will be supporting the successful applicant with training whilst in post.</p><p>These posts are fixed term post due to time

  <p>This role is offered on a permanent basis with the potential for flexibility in working hours and some remote working with the base being either Bury St Edmunds or Ipswich but with the expectation that you will work across both areas.</p> <p>The post holder will be an experienced Eating Disorder practitioner who, acting within their own professional boundaries, will provide care for adult service users with eating disorders. Supporting the Clinical Team Manager and leadership team, the post holder will actively participate in the day to day clinical and operational management of the Adult Eating Disorders Team within the locality, be part of a multi-disciplinary team and contribute to the development of the services they support. They will demonstrate safe clinical decision-making, and sound clinical expertise and knowledge of eating disorders and their treatments. The post holder will support the leadership team to meet the needs of the service users and contribute to service improvements and supervision of other members of the team.</p>

  <p>In partnership with Derby University, 4 successful candidates will be offered a full-time educational programme, funded at NHS Agenda for Change (AfC) Band 4 level, gaining credits towards a Post-Graduate Diploma qualification, plus employment on successful completion of training, with a starting salary projected to be on the NHS Band 5 scale. Your work will directly support recovery plans, focusing on key areas like managing emotions, improving sleep, and building confidence
Be a key part of the team - you'll work closely with your line manager, supervisor, and the rest of the drug and alcohol team, engaging with service users and their families to develop strengths-based, outcome-focused care plans
Grow your skills with expert guidance - you will receive regular supervision from a qualified professional, such as an HCPC-registered Clinical Psychologist or a BABCP-accredited CBT therapist. This ensures you're always learning and delivering the highest quality of care.
Collaborate and communicate effectively - you'll liaise with other health providers to ensure seamless continuity of care and communicate sensitive information to promote effective teamwork and positive outcomes for clients.</p>

  <p> Accept referrals via agreed protocols within the service. 
2 * Undertake patient-centred interviews which identifies areas where the 
person wishes to see change and or recovery and makes an accurate 
assessment of risk to self and others.

department's referral protocols, refers unsuitable clients on to the 
relevant service or back to the referral agent as necessary or steps-up

The post holder will split their time between university attendance and working in service, conducting assessments, and providing face to face and remote Low Intensity Interventions.

<h2>Job summary</h2>
  <p>The post holder will form an integral part of the multidisciplinary team approach in providingpersonalised care interventions to patients diagnosed with respiratory disease and their carers throughout the care treatment pathway. The Respiratory Support Practitioner will work closely with the Respiratory Specialist CNS's and Physiotherapists to support patients identified to the service by the respiratory outreach teams. They will support with all aspects of service provision, e.g.,training / equipment management, provide advice and support for the management of patients with respiratory disease. They will also support patients through the virtual ward hospital at home pathway by providing equipment and education to patients to support their discharge under early supported discharge pathway.</p>



  <h3>Main duties of the job</h3>
  <p>COPD/Asthma Bundles:1. Give advice and education to support management of their lung condition, including identifying that patients are on the correct inhaler and their technique is sufficient and selfmanagement plans are completed.2. Advice on pulmonary rehabilitation and appropriate making referrals.3. Education in recognising future signs of an exacerbation of their respiratory condition.4. Offer very brief advice on stop smoking and making referrals to the inpatient Tobacco Cessation Service.5. Collate and submit data to the National Respiratory Audit Programme (NRAP).6. Be able to perform ARTP accredited spirometry with reversibility.</p> <p><strong>Supportive discharges/Respiratory Virtual Ward:</strong>1. Take observations of patients and report back any concerns outside expected parameters.2. Visit patients in a community setting to perform observations and ensure patient is treated via the correct care pathway.3. Ordering of Oxygen for patients requiring it for discharge from hospital under the guidance of a prescribing clinician</p>
