  <p>We are looking for an energetic, highly motivated Clinical/Counselling Psychologist or CBT Therapist to join our dynamic Diabetes Psychology Team.</p> <p>If you are an enthusiastic and motivated clinician with experience in working with people with diabetes or Long-Term Conditions (LTC), and you are passionate about delivering evidence-based psychological interventions and service development, we encourage you to apply.</p> <p>This is an excellent opportunity to develop advanced clinical skills within a multidisciplinary team and be involved in a key service development project. Regular supervision will be provided.</p> <p>The post holder will be responsible for delivering a specialist psychology service to patients living with Type 2 diabetes in Well Street Common PCN and Hackney Marshes PCN.</p> <p>Your work within the Diabetes Psychology team will be central to improving patient engagement, self-management, and overall outcomes, with a specific focus on addressing health inequalities. This includes identifying and working with patients who struggle to attend appointments, as well as providing one-to-one and group-based support for individuals struggling with diabetes related distress. It will also involve wider outreach in the community and skilling up primary care staff to improve effectiveness of patient contact.</p> <p>You will work alongside a multidisciplinary team with disciplines including Endocrinology, Nursing, Psychology, Dietetics, and Diabetes Lay Educators and the wider primary care team.</p>

  <ul>  <li>Clinical: Providing specialist psychological interventions to complex patients living with Type 2 diabetes, within HM &amp; WSC PCNs to improve self-management and promote active, healthy lifestyles.</li>  <li>Service &amp; Community Engagement: Identifying barriers to engagement in diabetes management within the two PCNs through focused research, including questionnaires and focus groups.</li>  <li>Awareness &amp; Outreach: Engaging with the community to raise awareness of diabetes management resources, with a strong focus on marginalised groups.</li>  <li>Staff Training &amp; Support: Skilling up existing staff on the knowledge and awareness of psychological barriers to engagement in diabetes care, and utilising resources like MDMs to improve the effectiveness of patient contact and reduce disengagement.</li>  <li>Peer Support: Setting up peer support initiatives based on identified need, making strategic use of existing community resources.</li>  <li>Sustainability: Developing a sustainable model of care that can be fully integrated into the PCN's ongoing services after the initial funding period concludes.</li> </ul>

  <h3>Main duties of the job</h3>
  <p>The role requires a skilled clinician who can work both directly as well as indirectly to provide a range of clinical services and to manage and support psychology staff including senior psychologists, Clinical Associate Psychologists, Assistant Psychologists and other psychological professionals. You should have experience of working with service users with complex mental health including dual diagnosis and/or Autistic Spectrum Disorder presentations. Additionally, you will be familiar with the needs of local community of Newham and challenges of working with wider social determinants of health. </p> <p>You would be joining a dynamic psychological professionals' department within secondary care in Newham which offers a variety of evidence based interventions, as well as values practice based evidence, adapted therapies and co-production. You will have access to clinical supervision and professional management support from the Consultant Psychological Lead, and operational management support from a Senior Manager in community services</p>

<p>The West London CAMHS Eating Disorder Service is a well-established and expanding multi-disciplinary service which offers a range of evidence based clinical treatments to young people with eating disorders across the Boroughs of Ealing, Hounslow and Hammersmith and Fulham. The Service also links to a comprehensive Tier 2 and Tier 3 CAMH Service and a CAMHS Crisis Prevention/Care Team (ALLIANCE). The post holder will be part of a growing team of clinicians with a special interest and expertise in working with children, and young people, who are presenting with eating disorders.</p>
<p>The Dietician will also join a thriving team of supportive, committed and skilled nurses who are dedicated to changing the future outcomes for children and young people who present to mental health services.</p>



  <h3>Main duties of the job</h3>
  <p>The Dietitian will commit to participating in the CAMHS Development Programme, whilst also working within the Eating Disorder Service. For more information on the CAMHS Development Programme please see the Candidate Information Pack.</p>
<p>The Dietician will work within the intensive treatment community pathway. This is an exciting new development within the team to support young people to remain in the community or to provide consistent support on paediatric wards to support discharge. This role will provide outreach into the young person's home to and local wards to provide meal and more generalised therapeutic support.</p>
<p>The Dietitian will join the multidisciplinary team in the delivery of clinical assessment and provision of therapy to young people (0-18) and their families and/or carers within the Eating Disorder Service. The role will include providing outreach work to patients who may find it hard to attend clinics and in-reach work into Tier 4 Services. The Dietitian will provide specialist dietitian input. With supervision and support the Dietitian will provide individual therapy to identified patients, as well as short term therapy (individual, family, groups) to patients presenting with sub-threshold disorders. He /she will also contribute towards Multi Family Therapy Groups.</p>
