  <p>There is a rare opportunity due to staff moving on to join our Adult Acute Inpatient Mental Health therapies team as a horticultural therapist.</p>
<p>The post holder will provide individual and group assessment and intervention to service users with a range of mental health needs, providing an essential part of the therapy offer.</p>
<p>You will work alongside occupational therapists, physio, arts therapists as well as a full multidisciplinary team of medics, psychologists, nurses and HCAs.</p>
<p>There are regular opportunities for formal and informal training and skills development.</p>

We are looking for someone who has excellent communication skills, who is able to work within an emotionally demanding environment to deliver high quality and effective arts therapy provision across East London Community Forensic Service. We need someone who can work with a high degree of autonomy, who can use initiative in making decisions, is skilled in scoping and forming links with other services and who also works well within a variety teams. An ability to demonstrate a comprehensive understanding of managing risk and relational security when working with this client group is essential.</p> <p>The ELFT Community Forensic Services support service users in the process of and following discharge from the Forensic inpatient service, to reintegrate into the communities across the boroughs of Hackney, Newham, Tower Hamlets and North East London Foundation Trust (Redbridge, Barking Dagenham and Havering, and Waltham Forest).</p>



  <h3>Main duties of the job</h3>
  <p>The Community Forensic Service provides in-reach support to help service users move safely towards discharge and reduce length of stay in secure care; and enhanced support in the community, promoting sustained recovery and quality of life and reducing recalls to hospital. The service comprises several borough based multidisciplinary teams, including psychiatrists, FCPNS, Psychologists, Occupational Therapists, Family Therapists, Social Workers, Life Skills Recovery Workers and Peer Support and Care Peer Support Workers. The service model places a strong emphasis on multidisciplinary work and service user involvement, and co-production. The model stresses the importance of working across the care pathway from inpatient to community, and there is an emphasis on building relationships and signposting with community services (including community arts projects and arts in health providers) to work collaboratively to address the needs of the diverse population</p>

<p>The successful applicant will join the West London CAMHS Eating Disorder Service is based at in Ealing CAMHS, 1 Armstrong Way, Southall UB2 4SA.</p>
<p>The West London CAMHS Eating Disorder Service is a well-established and expanding multi-disciplinary service which offers a range of evidence based clinical treatments to young people with eating disorders across the Boroughs of Ealing, Hounslow and Hammersmith and Fulham. The Service also links to a comprehensive Tier 2 and Tier 3 CAMH Service and a CAMHS Crisis Prevention/Care Team (ALLIANCE). The post holder will be part of a growing team of clinicians with a special interest and expertise in working with children, and young people, who are presenting with eating disorders.</p>
<p>The Dietician will also join a thriving team of supportive, committed and skilled nurses who are dedicated to changing the future outcomes for children and young people who present to mental health services.</p>



  <h3>Main duties of the job</h3>
  <p>The Dietician will work within the intensive treatment community pathway. This is an exciting new development within the team to support young people to remain in the community or to provide consistent support on paediatric wards to support discharge. This role will provide outreach into the young person's home to and local wards to provide meal and more generalised therapeutic support.</p>
<p>The Dietitian will join the multidisciplinary team in the delivery of clinical assessment and provision of therapy to young people (0-18) and their families and/or carers within the Eating Disorder Service. The role will include providing outreach work to patients who may find it hard to attend clinics and in-reach work into Tier 4 Services. The Dietitian will provide specialist dietitian input. With supervision and support the Dietitian will provide individual therapy to identified patients, as well as short term therapy (individual, family, groups) to patients presenting with sub-threshold disorders. He /she will also contribute towards Multi Family Therapy Groups.</p>
<p>As a Dietitian you will have the opportunity to contribute to service development, audit, research and quality improvement projects and you will also act as a Supervisor for student dietitian on specialist placement in CAMHS.</p>
