Practices working together with a range of local providers to offer more
personalised and coordinated health and social care to the 70000 patient
population of Windsor in Berkshire.</p><p>We are looking to recruit to the post of Health and
Well-being Coach, to work within our Primary Care Network multidisciplinary
healthcare team.</p>



  <h3>Main duties of the job</h3>
  <p> Provide one-to-one coaching to
help patients identify goals and take steps towards improving their health and
wellbeing.</p><p> Support patients with
lifestyle change areas such as weight management, physical activity, healthy
eating, stress management, sleep, and smoking cessation.</p><p> Use behaviour change
approaches including motivational interviewing, active listening, and goal
setting to encourage patient confidence and self-management.</p><p> Deliver coaching remotely or
face-to-face across primary care sites.</p><p> Connect individuals with peer
support, self-management tools, and community activities to build confidence,
resilience, and social connection.</p>

approach to work with other members of the 13 strong PCN Health and Wellbeing

<h2>Job summary</h2>
  <p>We would like to hear from you if you can demonstrate your commitment to providing high quality care and counselling for depression at Step 3. You will be delivering counselling for depression using a stepped care model in line with the national IAPT programme and latest NICE guidance for psychological interventions. The role involves delivering one to one Counselling, Assessments and therapeutic groups.</p>
<p>You must demonstrate a commitment to Continuing Professional Development and be prepared to attend training in the Person Centred Experiential Counselling for Depression (PCE-CFD) IAPT training at Metanoia with a desirable commitment to Mind Matters of 1.5 years post training completion/award of qualification (not just attending the course).</p>
<p>Developmental Plan:</p>

<li>Maximum of 24 months to complete PCE-CfD training</li>

  <p>To provide a comprehensive assessment and therapeutic service for adult patients with common mental health problems referred to the IAPT SABP service based on sound, evidence based psychological principles, and working within the context of primary care services.</p>
<p>You will be able to assess clients for suitability for Counselling and other modalities, and provide specialist psychological treatment based upon clearly accepted, evidence-based principles (N.I.C.E. guidelines for Depression). To treat common mental health problems, including mood and adjustment disorders and other problems not of a severity requiring multidisciplinary specialist mental health services.</p>
<p>Be able to make decisions on suitability of new referrals, adhering to the department's referral protocols, and refer unsuitable clients on to the relevant service or back to the referral agent as necessary. To make an assessment of risk for patient and others and take appropriate action when necessary.</p>

  <p>We are looking for an energetic, highly motivated Clinical/Counselling Psychologist or CBT Therapist to join our dynamic Diabetes Psychology Team (30 hours per week).</p> <p>If you are an enthusiastic and motivated clinician with experience in working with people with diabetes or Long-Term Conditions (LTC), and you are passionate about delivering evidence-based psychological interventions and service development, we encourage you to apply.</p> <p>This is an excellent opportunity to develop advanced clinical skills within a multidisciplinary team and be involved in a key service development project. Regular supervision will be provided.</p> <p>The post holder will be responsible for delivering a specialist psychology service to patients living with Type 2 diabetes in Well Street Common PCN and Hackney Marshes PCN.</p> <p>Your work within the Diabetes Psychology team will be central to improving patient engagement, self-management, and overall outcomes, with a specific focus on addressing health inequalities. This includes identifying and working with patients who struggle to attend appointments, as well as providing one-to-one and group-based support for individuals struggling with diabetes related distress.
