  <h3>Main duties of the job</h3>
  <p> Provide one-to-one coaching to
help patients identify goals and take steps towards improving their health and

lifestyle change areas such as weight management, physical activity, healthy
eating, stress management, sleep, and smoking cessation.</p><p> Use behaviour change
approaches including motivational interviewing, active listening, and goal
setting to encourage patient confidence and self-management.</p><p> Deliver coaching remotely or
face-to-face across primary care sites.</p><p> Connect individuals with peer
support, self-management tools, and community activities to build confidence,
resilience, and social connection.</p>

approach to work with other members of the 13 strong PCN Health and Wellbeing

<h2>Job summary</h2>
  <p>An exciting and challenging opportunity has arisen for a permanent full-time band 6 Occupational Therapist within the Integrated Independence Team (Rapid Response Team).</p> <p>We are looking for an innovative, enthusiastic and experienced Occupational Therapist to be part of the Emergency department and community rapid response service within the Integrated Independence Team. This will be offered as permanent and full-time for an existing band 6 therapist or experienced band 5 therapist.</p> <p>The Integrated Independence Team is an integrated health and social care service provided jointly by Homerton University Hospital NHS Foundation Trust and London Borough of Hackney. The team places the user at the heart of the care planning process, and aims to promote independence, deliver outcome focused care, and provide a seamless journey through the care system.</p> <p>The team provides rapid response, intermediate care therapy and reablement services and consists of Occupational Therapists, Physiotherapists, Psychotherapists, Nurses, Speech and Language Therapists, Consultant Geriatricians, Social Workers, OT Technician, Care Support Coordinators, Care Support Workers, Therapy Assistants and Administration staff. The team will use integrated support planning to address health and social care needs across the pathway in a responsive, dynamic and flexible way.</p> <p>Working Hours: 37.5 hours per week (part time, flexible working or compressed hours will be considered).</p>



  <h3>Main duties of the job</h3>
  <p>We are looking for a Band 6 Occupational Therapist to provide a rapid response, senior Occupational Therapy assessment and treatment of complex patients within Emergency department, short stay unit and patients referred from the community in order to prevent admission to hospital and promote early discharge.</p> <p>In doing this you will work as part of a strong multidisciplinary team, within an interdisciplinary working model, to facilitate hospital discharge and prevent acute admission.</p> <p>The ideal candidate will have strong experience as an Occupational Therapist in an Acute Therapy or Rapid Response setting. You should have excellent communication skills, a proactive approach, prioritisation and problem-solving skills.</p> <p>You will be encouraged to develop your clinical, teaching and management skills through opportunities within the team and in the wider organisation. You will have support from a Clinical Specialist Lead and experienced staff across the team. Peer supervision and in service training are carried out on a regular basis and you will have an annual appraisal.</p> <p>There is excellent support for personal development and opportunities to develop the service in line with best practice. We offer a community service for pregnant and postnatal women with severe and complex mental health difficulties. The team has close links with other services, including IAPT, secondary care mental health teams and maternity as well as third sector organisations.   <p>Main Clinical Duties;</p>
<p>The main clinical work undertaken is to provide specialist psychological assessment and intervention for mothers and their babies and families in the community. The post holder will also offer clinical supervision to more junior staff and trainees.</p>
<p>The post holder will be involved in delivering evidence-based individual and group-based interventions. The psychological therapies team include clinical and counselling psychologists, as well as CBT therapists. The successful applicant will be part of a friendly and supportive team. There is an established reflective practice space for the whole team as well as regular individual supervision provided by the senior psychologist. Opportunities to develop supervision skills are consistently available and Oxleas has a longstanding relationship with clinical and counselling doctorate programs in taking trainees and developing the wider workforce.</p>

  <p>Community Mental Health Services have undergone widespread innovative transformation.
