We welcome applications from both newly qualified and experienced therapists.</p> <p>Community Rehabilitation supports people with mental health problems to live a full and active life within their family &amp; community, working in partnership with Social Care, Housing and Voluntary Community and Social Enterprise (VCSE) sector.</p> <p>As a key member of the multi-disciplinary team, you will be responsible for offering high-quality psychological treatments (individual &amp; group) and family interventions.   <p>We are seeking a compassionate and experienced Counsellor to join our multidisciplinary primary care team.
The successful candidate will provide short-term counselling interventions to patients aged 16 and over who are experiencing a range of emotional and psychological difficulties, with a strong emphasis on trauma-informed care.The post holder will work under the supervision of the Counselling Service Lead and the Primary Care Network Manager. Together, we care for over 37,000 registered patients.</p><p>The PCN was established to provide more localised support, expand access to services, and enhance the overall patient experience by meeting a broader range of needs and supporting practice capacity. We have a multidisciplinary team of clinicians who work under the mentorship and supervision of our General Practitioners, delivering care both within the practices and out in the community. This team includes Health and Wellbeing Coaches, a Dietitian, a Social Prescriber, and a Paramedic.
