  <p><strong>Band 4 Therapy Assistant Practitioner (TAP) OT/PT - 1 WTE</strong></p> <p>We are looking for a dynamic, enthusiastic and creative TAP to be a part of our well-established Occupational Therapy and Physiotherapy team based at Peace Children's Centre in Watford. You will be joining an innovative and agile Children and Young People's Therapy (CYPT) Service committed to delivering high quality intervention and transforming lives. </p> <p>Hertfordshire Community NHS Trust provides integrated Occupational Therapy, Speech and Language Therapy, and Physiotherapy Services for families across the county.   <h3>Main duties of the job</h3>
  <p>Children's Occupational Therapy and Physiotherapy is based on a 3 Tier model of service delivery, with provision at Universal, Targeted and Specialist tiers. We offer assessment and treatment to children and young people with a wide variety of conditions, including cerebral palsy, autistic spectrum disorder and developmental coordination disorder. We also carry out home assessments and advise on equipment, housing adaptations and rehousing. The team works closely with colleagues in the wider multiagency team supporting children, young people and their families to achieve their maximum potential.</p>
<p>An exciting development opportunity that is part of this role is that the postholder will support the service's moving and handling specialist to deliver manual handling advice and training to mainstream settings for CYP with physical disabilities. The postholder will receive full training to undertake this support role.</p>
<p>Continuing Professional Development is actively supported through access to training, regular supervision, peer support meetings and staff appraisal scheme.</p>

  <p>This post sits within the Children and Young Peoples Learning Disability Service within Mindworks at Surrey and Borders. The Children's LD Service is a Surrey County Wide Service, with 4 bases: Redhill Leatherhead Chertsey and Godalming. This post is being advertised as a development post, however if you have the relevant knowledge and experience for a Band 8a you are also able to apply.</p>
<p>The service is a friendly welcoming and supportive Team, made up of Nurses Psychology Music Therapy and Psychiatry along with Admin colleagues. There is a monthly Business Meeting and twice monthly clinical issues meeting, with a quarterly LD Forum.</p>
<p>The post holder will hold a case load of young people and provide appropriate interventions / support. This will include working with the family/ carers and wider MDT to ensure a consistency of approach. The post holder will also provide regular supervision to the AP based in the geographical area. They will be supported by the Clinical Psychologist in their development. Alongside regular supervision and appraisal with an identified development plan. The post holder will also contribute to the service ASD Pathway and any other pathways that are appropriate.</p>

<h2>Job summary</h2>
  <p>Psychologists are integral and valued key members of the Mental Health Care of Older People's multidisciplinary teams (MHCOP) in each borough across East London. Within and across the service, in line with National Guidance, the post holder will be responsible and accountable for the provision of high quality, person-centred and culturally sensitive psychological services to older adults presenting to the service, their families and carers. Our goal is to alleviate psychological distress and promote psychological well-being, working alongside our multi-disciplinary team colleagues in the community and liaison into the centralised wards, where required through consultation, teaching and training.</p>



  <h3>Main duties of the job</h3>
  <p>The post holder will be an active member of City and Hackney's Mental Health Care of Older Adult's service including the centralised MHCOP inpatient wards and wider East London MHCOP Psychology and Arts therapies professional group.</p> <p>They will work closely with psychologists and other professionals in the team maintaining a standardised approach to the assessment and care of Service Users with complex needs across community and inpatient settings. As part of this, they will contribute to the provision of a specialist psychological perspective for the work of the team, providing highly specialist psychological advice and consultation to other members of the team as well as consultation into the borough's residential and nursing homes.</p> <p>They will be responsible and accountable for the systematic provision of a highly specialist psychology service for people with severe, complex and enduring mental health and dementia related difficulties and their families, presenting to the service and across community and, where appropriate, acute physical health and psychiatric settings.</p> <p>They will develop working partnerships with other members of psychology services locally and across East London boroughs, relevant statutory and voluntary agencies, locality services and providers/commissioners as appropriate</p>   <h3>About us</h3>
