Are you a dynamic and motivated individual with strong leadership skills? Ifsothe Bolton Home Based Treatment Team is seeking to recruit aSenior Urgent Response Practitioner at Band 7to provideClinical Leadershipworking alongside the Team Manager supporting service delivery and development.

The Home Based Treatment Team is an established service andcomprises ofa dynamic and welcoming team of psychiatrists, senior practitioners, practitioners as well as a psychological therapist. The team has senior level input into the local and GM strategic vision for Home Treatment services.

TheHBToperates 24/7 to provide responsive mental health care in the community setting, delivered by experienced andestablishedmultidisciplinary teams. We aim to provide an alternate to hospital admission and care for service users in the least restrictive way.

TheSenior Practitionerwill develop close links with the Inpatient Wards to promote HBTT interventions with service users with the view to supporting earlier discharge from the inpatient wards aiming to reunite service user with their families / carer whilst also maintaining HBTT support in the community.

Provide clinical leadership, line management supervision, support and consultation to HBT practitioners, in line with the clinical pathway as outlined within the operational procedure.

Manage and evaluate a caseload in line with the clinical pathway, as outlined within the operational procedure.

This will include: Assessment - Formulation - & Collaborative care planning as

  • Therapeutic interventions as required
  • Signposting to other relevant agencies and services as part of their recovery
  • Working with the service users and their carers as part of their journey through HBT
  • Play an active part in service development and innovation as identified by the directorate and through leadership meetings.
  • Build and maintain good working relationships with other agencies both in and out of the Bolton area, to ensure the best care delivery for the service user and their carers and/or families.

The post holder will be required to work 37.5 hours a week including shifts, nights, weekends and bank holidays.

The post holder would be expected to carry a clinical caseload, and support the staff team with managing complex cases, as well as play an active part in the leadership and service development of HBT.

There is an expectation that the post holder will participate in out-of-hours management on call rotas in providing management cover for weekends and statutory holidays.
