Key Responsibilities

  • Manage and coordinate a caseload of individuals with mental health and dementia-related needs.
  • Conduct comprehensive assessments and develop person-centred care plans.
  • Deliver evidence-based nursing interventions and monitor patient outcomes.
  • Administer and manage medications in accordance with professional and organisational guidelines.
  • Assess and manage clinical risks, promoting safe and effective care.
  • Work collaboratively with multidisciplinary teams, carers, and external agencies.
  • Maintain accurate clinical records and ensure compliance with relevant policies and standards.
  • Provide clinical supervision, guidance, and support to junior staff and students.
  • Participate in service development, quality improvement, audits, and research activities.
  • Deliver health promotion, education, and recovery-focused interventions.
  • Support safeguarding practices and ensure the wellbeing of vulnerable adults.
  • Demonstrate leadership, professionalism, and a commitment to high-quality patient care.

Essential Requirements

  • Registered Mental Health Nurse (NMC Registered).
  • Degree in Nursing or equivalent qualification.
  • Significant post-registration experience in mental health services.
  • Experience working within community mental health settings.
  • Strong assessment, risk management, communication, and care coordination skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Commitment to continuous professional development and evidence-based practice.
