Key Responsibilities
- Manage and coordinate a caseload of individuals with mental health and dementia-related needs.
- Conduct comprehensive assessments and develop person-centred care plans.
- Deliver evidence-based nursing interventions and monitor patient outcomes.
- Administer and manage medications in accordance with professional and organisational guidelines.
- Assess and manage clinical risks, promoting safe and effective care.
- Work collaboratively with multidisciplinary teams, carers, and external agencies.
- Maintain accurate clinical records and ensure compliance with relevant policies and standards.
- Provide clinical supervision, guidance, and support to junior staff and students.
- Participate in service development, quality improvement, audits, and research activities.
- Deliver health promotion, education, and recovery-focused interventions.
- Support safeguarding practices and ensure the wellbeing of vulnerable adults.
- Demonstrate leadership, professionalism, and a commitment to high-quality patient care.
Essential Requirements
- Registered Mental Health Nurse (NMC Registered).
- Degree in Nursing or equivalent qualification.
- Significant post-registration experience in mental health services.
- Experience working within community mental health settings.
- Strong assessment, risk management, communication, and care coordination skills.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to continuous professional development and evidence-based practice.
