Part-Time Community Mental Health Practitioner-Recovery

Kent and Medway NHS and Social Care Partnership Trust is seeking part time Community Mental Health Practitioners in Sheerness to join a multidisciplinary team. The post requires RMN, OT or Social Work registration and focuses on community-based mental health care for adults 18+.

You will assess, plan, deliver and evaluate care with a recovery oriented approach. Role involves holistic assessments, care planning with service users and carers, and education to promote self management and wellbeing.
