Neighbourhood Health Prescriber - Lead Clinician

Cardiff and Vale University Health Board is seeking a Neighbourhood Health Practitioner (Prescriber) to work autonomously across primary care and community settings, delivering advanced clinical assessment, diagnosis, treatment and prescribing for patients with complex needs.

You will lead the neighbourhood MDT, supervise care coordinators, develop personalised care plans and use population health data to target early interventions, aiming to reduce hospital admissions and health inequalities.
