This role is offered on a permanent basis. The hours are part-time, 22.5 hours per week.

We are delighted to offer an exciting opportunity for a motivated and compassionate Community Support Worker to join our friendly and dedicated Cheltenham Recovery Team. This part-time Band 4 role is ideal for someone who is passionate about supporting adults living with severe mental illness to regain stability, independence, and an improved quality of life.

As a key member of our multidisciplinary team, you will work closely with service users, placing their needs and aspirations at the heart of every intervention. You'll provide practical and emotional support, helping individuals develop confidence, learn new skills, and engage with their communities in meaningful ways.

You will collaborate with a wide range of internal colleagues--such as nurses, occupational therapists, psychologists, and peer workers--as well as external health and social care partners, ensuring a coordinated and person-centred approach to recovery. No two days are the same, and you will play an essential role in delivering high-quality community-based care.

Main duties of the job

  • Provide care coordination for an allocated caseload of service users, ensuring their needs are assessed, reviewed, and met through personalised care plans.

  • Deliver goal-focused, recovery-oriented interventions to support individuals in developing skills, confidence, and independence.

  • Facilitate and contribute to group work and therapeutic clinics, supporting service users to engage in structured activities that promote wellbeing.

  • Work collaboratively with internal team members and external partners to ensure high-quality, person-centred care.

  • Maintain accurate, timely documentation in line with professional and organisational standards.

  • Participate in multidisciplinary discussions, contributing to risk assessments, care planning, and holistic treatment approaches.

  • Share your knowledge, experience, and skills with colleagues, students, and new staff, supporting a culture of learning and continuous improvement.

  • Promote service user involvement and uphold values of dignity, respect, and empowerment in all aspects of care

The Band 4 Community Support Worker plays a vital role within the Cheltenham Recovery Team, supporting adults living with severe and enduring mental health conditions to achieve meaningful recovery goals and improve their quality of life. Working as part of a supportive and multidisciplinary team, the post-holder delivers person-centred, community-based interventions, ensuring service users are actively involved in their own care and treatment.

This part-time role offers a unique opportunity to make a significant difference in peoples lives while developing your own professional skills. You will work closely with internal colleagues and external partner agencies, helping to coordinate care, promote independence, and encourage engagement with a range of therapeutic and practical support services. Ongoing development, supervision, and training are integral to the role, supporting you to progress your career within mental health services.

Main Responsibilities
  • Care Coordination: Hold a small caseload of service users, completing assessments, developing person-centred care plans, and regularly reviewing progress.

  • Deliver Goal-Focused Interventions: Provide structured, recovery-oriented support that helps individuals build confidence, develop coping strategies, and work toward personal goals.

  • Support Groups and Clinics: Contribute to the running of therapeutic groups, psychoeducation sessions, and community clinics, encouraging service user participation and engagement.

  • Collaborative Working: Liaise effectively with internal team members and external health and social care providers to ensure coordinated, holistic care.

  • Risk Assessment & Management: Participate in risk assessments and implement risk management plans, ensuring safety and wellbeing remain central to all work.

  • Record Keeping: Maintain accurate, up-to-date documentation in line with organisational and professional standards.

  • Professional Development: Engage in supervision, reflective practice, and training opportunities to enhance skills and support career progression.

  • NVQ level 3 or equivalent level of competency-based training in a relevant health or social care area PLUS theoretical and practical knowledge to diploma level equivalent, obtained through formal competency training, short courses and experience OR successful completion of approved associate practitioner training or relevant foundation degree or commitment to obtain within 2 years.
  • IT Skills

LENGTH AND / OR NATURE OF EXPERIENCE

Essential

  • Experience in mental health or social care services at Bands 2, 3 or 4, ideally in a community setting.
  • Knowledge of the CPA process
