Bolton Community Mental Health Services are a committed integrated service that strives to provide quality recovery based support to people with severe and enduring mental health related difficulties in Bolton 7 days per week, 365 days per year. The service calls upon a wide range of evidence based holistic assessments and interventions.

This job description is not exhaustive, but is intended to give an overall picture of the role. Other duties within the general scope of the post may be required from time to time.

Responsibilities
  • Provide skilled mental health nursing care to people in the community.
  • Assume the role of care coordinator for a complex caseload.
  • Work as a member of a multi-disciplinary team.
  • Have a theoretical and working knowledge of the Safeguarding Adults and Children's framework, and take the lead role when necessary.
  • Undertake initial assessments of people referred into the service, linking directly with the Team Leader and wider MDT.
  • Develop, implement and evaluate holistic programmes of care, including assessment of carer's needs.
  • Maintain close links with other statutory and non statutory professionals and agencies, and access resources from provider services.
  • Act as assessor/mentor for student nurses.
  • Work proactively and flexibly within a multi-disciplinary team model, outside normal office working hours.
