Community Mental Health Practitioner - Recovery-Focused Care

Lancashire and South Cumbria NHS Foundation Trust is seeking a Band 5 Community Mental Health Practitioner for Chorley CMHT. You will join a friendly, multidisciplinary team delivering person-centred care and working with service users, families and partner organisations.

You will undertake assessments, develop care and safety plans, and manage a caseload with complex needs while contributing to clinical reviews and service development.
