Community Mental Health Practitioner - Older Adults (Flexible)

Lancashire and South Cumbria NHS Foundation Trust is seeking an experienced Community Mental Health Practitioner to join our busy Community Mental Health Team for Older Adults in South Lakes.

As a Assessment and Duty Practitioner you will provide assessment, advice and treatment for people with complex mental health needs, undertake triage and duty responsibilities, and support the team's integration with memory services, care home liaison and physical health provision.
