Job summary
We are looking for an expert by experience that has lived experience of being a carer of someone who has mental health issues. The Peer Support Carer's Lead works collaboratively and aims to assist in improving the wellbeing of families and carers of people who are admitted to the Crisis Resolution and Home Treatment Team by supporting carers directly, either via 1:1 or groups, developing and maintaining relationships with the service provider teams, providing carers with updates of their relatives care and treatment, contributing to service development and research.
Main duties of the job
We support patients, family members and carers and remain committed to the recovery model and a holistic way of working in their homes, offering a variety of interventions and support to aid recovery.
To initiate, establish and maintain relationships with families and carers in a variety of settings, while developing trust and rapport.
Act as a coach and mentor to families and carers by providing information about support and services available for them, ensuring they have carers communication plans where required.
Work collaboratively with inpatient/CRHT/ATS to support carers via 1:1 support or group settings.
To work within The Triangle of Care standards and undertake self assessments and action plans.
