Care Coordinator - Community Psychiatric Nurse
We are looking for an enthusiastic Community Psychiatric Nurse (CPN) to join our friendly multi disciplinary team. The role requires experience in mental health settings and awareness of community mental health care delivery.
As a CPN you will hold and manage a caseload of people and have care coordinating responsibilities under the Care Program Approach Guidelines. You will assess clients and their carers' needs and access & signpost to a wide range of community resources and provider services available in the local area.
Main duties- Assessments, care planning, psychosocial interventions, safeguarding, duty assessments, medication administration.
- Report writing, attending tribunal hearings, supporting depot clinic.
Complete all duties as outlined in the attached job description and person specification.
Benefits- Pay Enhancements - 30 % additional for evenings (8 pm onwards) and Saturdays, 60 % additional for Sundays and bank holidays.
- 27 days annual leave plus bank holidays, rising to 29 after 5 years and 33 days after 10 years.
- Full time, 37.5 hours per week with flexible working options.
- Pro rata salary between £39 959 - £48 117 per annum (Yearly).
- Experience of working in a community mental health setting.
- Safeguarding enquiry lead experience.
- Funding application/social care/CCG experience.
- Experience using electronic recording systems.
