  <p>An exciting and challenging opportunity has arisen for a permanent full-time band 6 Occupational Therapist within the Integrated Independence Team (Rapid Response Team).</p> <p>We are looking for an innovative, enthusiastic and experienced Occupational Therapist to be part of the Emergency department and community rapid response service within the Integrated Independence Team. This will be offered as permanent and full-time for an existing band 6 therapist or experienced band 5 therapist.</p> <p>The Integrated Independence Team is an integrated health and social care service provided jointly by Homerton University Hospital NHS Foundation Trust and London Borough of Hackney. The team places the user at the heart of the care planning process, and aims to promote independence, deliver outcome focused care, and provide a seamless journey through the care system.</p> <p>The team provides rapid response, intermediate care therapy and reablement services and consists of Occupational Therapists, Physiotherapists, Psychotherapists, Nurses, Speech and Language Therapists, Consultant Geriatricians, Social Workers, OT Technician, Care Support Coordinators, Care Support Workers, Therapy Assistants and Administration staff. The team will use integrated support planning to address health and social care needs across the pathway in a responsive, dynamic and flexible way.</p> <p>Working Hours: 37.5 hours per week (part time, flexible working or compressed hours will be considered).</p>



  <h3>Main duties of the job</h3>
  <p>We are looking for a Band 6 Occupational Therapist to provide a rapid response, senior Occupational Therapy assessment and treatment of complex patients within Emergency department, short stay unit and patients referred from the community in order to prevent admission to hospital and promote early discharge.</p> <p>In doing this you will work as part of a strong multidisciplinary team, within an interdisciplinary working model, to facilitate hospital discharge and prevent acute admission.</p> <p>The ideal candidate will have strong experience as an Occupational Therapist in an Acute Therapy or Rapid Response setting. You should have excellent communication skills, a proactive approach, prioritisation and problem-solving skills.</p> <p>You will be encouraged to develop your clinical, teaching and management skills through opportunities within the team and in the wider organisation. You will have support from a Clinical Specialist Lead and experienced staff across the team. Peer supervision and in service training are carried out on a regular basis and you will have an annual appraisal.</p> <p>There is excellent support for personal development and opportunities to develop the service in line with best practice. You will have the opportunity to support and supervise junior staff within the service, and to support service development and projects with the team.</p>

  <p>Community Mental Health Services have undergone widespread innovative transformation. North London Foundation NHS Trust is at the forefront of this transformation, developing a more holistic, recovery oriented, biopsychosocial approach to supporting people with mental health difficulties, in partnership with primary care, other community NHS providers and the voluntary sector. These relationships will be central to the new community mental health model.Clinical Associate Psychologists are an innovative role in mental health services, that are core to this community transformation and offering high quality clinical care to the Camden borough.We have an opportunity for a qualified Clinical Associate Psychologist (Band 6) to join our South Camden Core Team on a full-time basis.   <h3>Main duties of the job</h3>
  <p>The CAP role is central to the team's psychological provision, with opportunities for group interventions, 1:1 therapy using a Structured Clinical Management model and other evidence based approaches, alongside joint assessment, formulation, intervention and consultation.</p> <p>There will be opportunities for leadership, research, service development and consultation to the system, which will involve liaising with professionals involved in the in care of the individual. The teams work closely as MDTs, with weekly clinical meetings where complex cases can be thought about and MDT formulations can be developed. Senior Paediatric Audiologist

  <p><strong>An exciting opportunity has arisen to join our dynamic therapy team. We are looking for a part time position (3 days a week) for a Speech and Language Therapy Assistant within our Adult SLT Service. We work with patients in their own homes, in clinics at inpatient neuro-rehabilitation and in Lister Hospital.</strong></p>
<p>The post-holder will need to be organised, enthusiastic, a good communicator and will have previous experience in a rehabilitation or care delivery setting. You will be assisting in the rehabilitation of patients with a wide range of neurological needs (including physical, communication and cognitive) which may include both individual sessions and group work. You will be working closely with the Speech and Language Therapists and a wider MDT including the medical team, Physiotherapists, Occupational Therapists, Psychologists and Nurses. You will need to be able to keep accurate clinical records and have good clerical/ administrative skills.</p>

  <p>The post-holder will be responsible for supporting Speech and Language Therapists with the assessment and management of adults with acquired communication, speech, language or eating and drinking disorders.</p>
<p>You will carry out therapy plans with patients who may have a range of diagnoses including stroke, Parkinson's, dementia, Motor Neuron disease and other progressive neurological conditions. You will need to build excellent rapport with patients and providing them with a high level of care.</p>
<p>You will need to be able to keep accurate clinical records and have good clerical/ administrative skills.</p>
<p>You will support the team with resource building, such as low tech augmentative and alternative communication aids (AAC).</p>
<p>You will help to run communication groups alongside other SLTAs.</p>

As a Place2Be Supervising Counsellor- Peripatetic you will provide quality service in schools in situations such as new school set up, cover School Based Staff long and short term absences and extra support when required.
