  <p>The post holder will be working within a busy team alongside a team ofregistered mental health nurses and be supported by the Team Manager,Senior Clinical Lead/CBT Practitioner, and team administrators.</p> <p>Post holders will have a high level of autonomy working across designated GP practices, managing their own clinics and providing mental healthassessment and evidenced based treatment for people aged over 18. You will provide a service to people who exhibit complex common mental health needs and stable, predictable and enduring serious mental health needs. You will be expected to develop and manage all aspects of their service, such as waiting lists according to local need, working within the parameters of the service operational policy.</p> <p>A key aspect of this role is GP consultation, liaison, education and training. You will be expected to attend GP practice meetings at least twice a year to discuss the service and actively seek feedback and report any issues to the team leader as well as areas for improvement and any innovative practice for sharing and disseminating. Staff will be expected to autonomously manage high volumes of referrals. Depots to be administered by practice nurses. The successful candidate for this varied and dynamic role will be delivering 1:1 assessments and interventions for individuals with learning disabilities.   <p>The successful candidate will be part of a team committed to MDT working with people with learning disabilities. You will hold an individual caseload for delivering 1:1 interventions within the Wolverhampton locality, working systemically in line with families and carers where appropriate. With support from psychology and MDT colleagues you will lead on developing, delivering and evaluating a Division wide group programme to ensure citizens with a learning disability have appropriate access to evidence based psychological therapy. You will be skilled in both individual and group work adapted to match the different intellectual &amp; emotional abilities within the client group. </p>  <p>The team provides a full range of assessments, interventions, support, training and advice to individuals themselves, their families and paid carers and the network of multi-agency stakeholders who are involved in the care and support of people experiencing complex behavioural and/or emotional distress. A core member of the Wolves CLDT, you will bring experience combining the best of evidence based clinical assessment &amp; formulation with client centred therapeutic interventions.   <p>In partnership with Derby University, a successful candidate will be offered a full-time educational programme, funded at NHS Agenda for Change (AfC) Band 4 level, gaining credits towards a Post-Graduate Diploma qualification, plus employment on successful completion of training, with a starting salary projected to be on the NHS Band 5 scale. <p>To work in a custodial environment delivering a high quality drug and alcohol recovery service in response to need.</p>
<p>To provide a range of psycho-social interventions motivating and supporting people to achieve recovery and positive outcomes within a recovery-orientated substance misuse treatment service.</p>
<p>To work in collaboration with clinical prescribing services to ensure the needs of each service user is met.</p>
<p>To provide a drug and alcohol recovery service (incorporating assessment, recovery planning, casework coordination and psycho-social interventions) to people located within the prison establishment.</p>
