Health coaches do this by coaching and motivating patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives and providing interventions such as self-management education and peer support.</p><p>The successful candidate will be kind, reflective and self-aware and will enjoy working with a wide range of people.   <p>We have an exciting opportunity for an enthusiastic and motivated Band 3 Rehab support worker to join our therapy team working in the bed-based rehabilitation unit at Tiverton and District Community Hospital. Registered Mental Health Nurse

<p>This role includes a welcome bonus of £5000, with the added possibility of extending the workdays from three to four.</p> <p>Are you an experienced Psychologist looking for a rewarding career where you can make a difference and changes lives for the better?
