  <p>The Chronic Pain Self Management Team is a well established and experienced multi disciplinary team offering highly specialist assessment and intervention for people living with complex chronic pain presentations. We aim to support people referred to our service to develop a range of self management skills to allow them to improve their overall quality of life despite pain. Interventions are focussed on improving understanding of chronic pain, increasing function and engagement in valued activities, managing pain related distress and reducing inappropriate health case. </p> <p>This post would suit a Physiotherapist who is looking to increase their skills in chronic pain / long term condition management using a biopsychosocial framework. Support will be provided to assist the post holder to -o Increase their knowledge of complex chronic pain presentationso Deliver self management interventions in groups and with individualso Develop knowledge and skills in delivering approaches based on CBT/ ACT / mindfulness models</p> <p>Specialist supervision and support will be provided by senior physiotherapists in the team. </p>



  <h3>Main duties of the job</h3>
  <p>To provide a high standard of service to patients attending the Chronic Pain Service working as an autonomous practitioner, with advice and support from Band 7 and 8a Physiotherapists, the service Clinical Lead and other team members. The post holder will work with patients presenting with complex chronic pain presentations, offering specialised clinical assessment, treatment planning and appropriate discharge from the service, adapting practice to suit the needs of the patient. </p>

<h2>Job summary</h2>
  <p>The post holder will be based at Monnow Vale Health &amp; Social Care Facility, Integrated Service Hub for Central Monmouthshire, workingalongside health &amp; social care colleagues in a well-established community based Integrated Team. The post holder will lead on thedevelopment &amp; delivery of a specialist falls service for Tier 3 Complex Fallers, engaging the wider MDT, and working alongside existingqualified and support staff, and support delivery of a physiotherapy service to Tier 2 Fallers requiring Reablement input.The postholder will additionally manage a general community-based caseload as required</p> <p>The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.</p>



  <h3>Main duties of the job</h3>
  <p>Provide advanced clinical falls risk assessment, clinical reasoning and diagnostic skills, for older people with a wide variety of complexfalls presentations for service users with unexplained falls, dizziness and giddiness in a variety of community settings, including HMPUsko Devise bespoke intervention management and rehabilitation programmes for this specialist caseload based upon the findings of thespecialist assessment. This may include vestibular assessment and treatment and devising CBT programmes for service users fearfulof fallingo Independently manage this highly specialist complex caseload, making differential diagnosis and advising on further medicalintervention/Investigation required in a timely mannero Provide a wide range of clinical and leadership skills to support management of the Reablement Physiotherapy caseload and directmanagement of Tier 2 fallers (with a diagnosis predisposing to falls)</p>
