  <p>We are looking for a substantive Band 6 Forensic Community Mental Health/LDA Nurse Practitioner for the FIND Team and Forensic Outreach and Liaison Service (FOLs).</p>
<p>The Forensic Intellectual, Neuropsychiatric &amp; Developmental Disorders (FIND) team and Forensic Outreach and Liaison Service (FOLs) are expanding services that with an imminent expansion are going to merge, that provide support and intervention to service users transitioning from secure care to the community. The teams are based at Fromeside, Blackberry Hill Hospital. The teams provide cover across BNSSG, BSW, Somerset and Gloucester</p>
<p>We also offer specialist advice and guidance to mental health and other relevant services.</p>
<p>The successful applicant will care coordinate a small caseload with support from an expert MDT and jointly work with recovery teams.</p>
<p>This is an exciting post in an innovative developing service and we are aiming to develop a highly skilled team. There are training opportunities in a range of therapeutic skills and clinical tools, including, HCR- 20 and Dundrum.</p>
<p>We are looking to recruit a dynamic, motivated and experienced Registered RMN/LD Nurse. You will need to have a minimum of 2 years post qualification experience in mental Health</p>



  <h3>Main duties of the job</h3>
  <p>This opportunity would be perfect for someone who has relevant forensic experience with a keen interest in learning disabilities and autism.</p>
<p>This role is focused on providing appropriate, effective interventions and treatments, including administering depots to people with Forensic LD&amp;A needs, their carers (including friends and relatives) and their supporters in the community, enabling and assisting them to meet daily health, social care and well being needs, in line with personal recovery goals, and facilitating engagement with mainstream services. The post holder may also be required to provide support, advice and supervision to team members and other mental health and CLDT teams.</p>
<p>Always under the overarching framework of CPA, the post holder will be responsible for the ongoing assessment, planning delivery and review of activities and interventions against identified health, social care and well being needs, acting as care coordinator for a defined group of service users. The role will also require undertaking and delivering specific health or social care assessments and interventions, including identified service users on other caseloads. This maybe either on a one to one basis, or as part of a group activity.</p>
<p>It is an essential requirement that applicants have a driving licence and their own personal transport.</p>
