As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week. We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital.</p> <p>We are a team that consists of Band 5 and 6 Nurses, Band 3 Support Workers, Non-Medical Prescribers, Medical Staff, Occupational Therapists, Social Workers, Administrators and Business Support Staff. </p> <p>As we work with people with all types of mental health presentations, we need to have a range of skills and interests that create a strong and responsive team.</p>



  <h3>Main duties of the job</h3>
  <p>Mental Health Practitioner - Nurse, OT, Social Workers</p> <p>Working in a multi-disciplinary team and in partnership with internal and external agencies, you will;</p> <ul>  <li>Provide specialist assessment and care planning in conjunction with service users and carers</li>  <li>Evaluate treatment plans and undertake home treatment</li>  <li>Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues</li>  <li>Co-ordinate the work of the Team</li> </ul>

  <p><strong>Please note that this job does not meet the salary requirements for work visa sponsorship and as such we will be unable to progress your application if you do not have the right to work in the UK.</strong></p> <p>An excellent opportunity has arisen for Band 3 Rehabilitation Technician with previous reablement experience in a community or acute setting to join the Central Team, Shropshire.</p> <p>You will join a multidisciplinary team, working with Physiotherapists, Occupational Therapists, Assistant Practitioners and Rehabilitation supporting patients in a variety of settings within their own homes or care homes.</p> <p>You will work under guidance to support patients with a range of conditions, provide rehabilitation, and working with the team to support rehabilitation and short/long term management planning.
