Our alliance model is aligned with the government’s neighbourhood health agenda – enabling the NHS to work in a fully integrated way with a range of community based and grassroots organisations. Ensuring that data flows from these organisations is critical.<br><br>The role will build a robust data model, work directly with voluntary sector partners to understand their CRM systems, and develop automated submissions that ensure compliance with statutory requirements.   <p>Working within the MHIR team at HMP Downview, you will be working within a supportive team within our mental health hub. You will benefit from having peer support as you go through your induction phase to aid familiarity with how the prison functions. You will have opportunities to be part of local service developments and we promote all team members to become a specialist lead in relevant areas. You will also benefit from our inclusive approach. In addition to mandatory training opportunities, we also consider external training development, there is Mental Health Practitioner peer supervision and monthly Reflective Practice which is in addition to 1:1 supervision.</p>

  <p>An opportunity has arisen for a dynamic, forward-looking Band 6 occupational therapist to join our Adult Eating Disorders Inpatient ward working alongside and occupational therapy apprentice. You will have experience in mental health and occupational therapy and whilst desirable, eating disorder experience is not necessary as full support will be given to develop expertise in this area. This is a great opportunity for as existing Band 6 looking to widen their skills or a Band 5 OT looking to progress their career.</p> <p>You will be part of a wider occupational therapy team within the eating disorder service and provide specialist assessments and plan and implement treatment for people with severe eating disorders which will include eating with clients on the unit and the community.</p>   <h3>Main duties of the job</h3>
  <p> To manage a caseload of clients with complex psychological and physical needs, using evidence based/client centred principles to assess, plan, implement and evaluate interventions.</p> <p> To provide outcome-oriented group and individual occupation-focused interventions including community work.</p> <p> To provide safe, effective, quality OT interventions that ensure positive client experience.</p> <p> To provide generic keywork or hold a link worker role for a specified number of clients.</p> <p> To provide carer interventions.</p> <p> To work within other parts of the service as required.</p> <p> To participate in the planning, development and evaluation of OT services within the eating disorders service, leading on delegated projects.</p> <p> To act as the designated 'Disability Champion' leading the multi-disciplinary team in Disability Equality matters within your area and other areas as required.</p> <p> To contribute to the maintenance and development of the Trust OT Service</p> <p> To support OT students on practice placement.</p> <p> To promote recovery and social inclusion.</p> <p> To take responsibility for managing clinical risk and single-handedly implementing OT treatment</p>

Occupational Therapist (Avalon Ward)

<h2>Job summary</h2>
  <p>The Genomic Associate will be based in the Clinical Genetics Service at Northwick Park Hospital and will work alongside clinical geneticists, genetic counsellors, laboratory staff and other healthcare professionals to provide enhanced contact with patients at the point of referral, and after genetic testing and counselling, to facilitate the collection of information and obtain consent, where needed. The genomic Associate will also help to facilitate access to the service, particularly amongst marginalised communities and families who are difficult to engage. The post holder will be responsible for requesting test reports, relevant tissue/samples and testing by liaising with laboratories, families, and clinical teams, as well as the tracking of such requests They will keep clinicians and patients updated on progress, highlighting any long waiters.</p> <p>Under supervision, you will support the team with keeping spreadsheets, databases and portals updated for gene carriers, as well as undertaking regular audit to ensure patients are accepted onto screening programs. They will be responsible for arranging breast screening for those at high risk and keeping spreadsheets/records updated.</p> <p>the post holder will support the team with cancer triage by obtaining family history information, entering of data for online cancer risk assessments and generating a risk calculation using that program (e.g. CanRisk) for interpretation and use by genetic counsellors and clinical geneticists.</p>



  <h3>Main duties of the job</h3>

<p>Contact healthcare professionals, laboratories, and patients to gather referral details, family history, and genetic test reports.</p>

<p>Provide pre-clinic information, verify consent requirements, and explain the need for relatives' reports where appropriate.</p>

<p>Check variant pathogenicity using databases and liaise with laboratories for reviews.</p>
</li>
<li>
<p>Track and manage tissue or genetic test requests, keeping clinicians and patients updated on progress or delays.</p>
</li>
<li>
<p>Maintain spreadsheets, databases, and screening programme records; complete referral forms under supervision.</p>
</li>
<li>
<p>Liaise with screening services, support audits, and respond to information requests from other centres.</p>
</li>
<li>
<p>Communicate compassionately with patients and relatives, recognising distress, health literacy needs, or learning difficulties.</p>

<p>Assist with obtaining blood or saliva samples, support clinicians with documentation, and escalate ethical concerns when needed.</p>
