<h2>Job summary</h2>
  <p>We
are recruiting a full-time Band 6 Specialist Practitioner and Supervisor
(trainee or trained) to join Children and Family Health Devon within the Mental
Health Support Teams (MHST) and Child Wellbeing Practitioner (CWP) services.</p><p>The
role supports an early intervention pathway for children and young people
experiencing mild to moderate anxiety and low mood, and contributes to
whole-school approach work in education settings. For trainee applicants, there
is an opportunity to complete the PGCert in Clinical Supervision of
Psychological Therapies Practice at the University of Exeter.</p><p>MHSTs
across Devon and Torbay include Education Mental Health Practitioners (EMHPs)
and CWPs who deliver evidence-based interventions in schools. The post-holder
will provide clinical supervision, support staff practice, and help maintain
consistent and safe care. Further information about the national MHST model is

Wellbeing Practitioner (CWP) the opportunity to join our service on 1 year
fixed term contract. The CWP team sits within our Mental Health Support Teams
(MHST) early intervention pathway. We are looking for practitioners with
proactive and positive attitudes to support our waiting lists, so that children
can be seen quicker. The successful candidates will join our CWP team to work
with children presenting with mild-moderate presentations of mental health who
require Low Intensity- Cognitive Behavioural Therapy (LI-CBT). The role may
involve some travel around the county, seeing young people in community and
clinic settings and so having a driving license and car will be essential to

Community Health Care Support Worker

<h2>Job summary</h2>
  <p>The Crisis Resolution Home
Treatment Team is a pivotal service whose main focus is to offer an alternative
to an inpatient admission and provide intensive treatment as required within
the community. The service will also ensure effective gatekeeping of all
service users referred to the service for admission to Hospital. The service
will operate on an outreach basis with the majority of assessments taking place
in non-Hospital
environments.</p><p>We are currently recruiting a Community Support worker for the Teignbridge CRHTT for 22.5 hours a week.</p><p>

The post holder will be an active member of the
integrated multi-disciplinary team (MDT),
providing a 24-hour community-based Crisis Resolution and Home Treatment
Service as an alternative to hospital admission.</p><p>As part of a MDT, the Community Support Worker is a pivotal role in shaping, implementing and facilitating interventions for individuals with a diverse mental health needs within the community.</p><p>Under the direction and leadership of a qualified Mental Health Professional, the team are responsible for:</p><ul><li>Deliver a collaborative care plan with specific home interventions and goals.</li><li>Offer crisis resolution as an alternative to hospital admission, available 24 hours per day, 7 days a week, 365 days per year.</li><li>Serve as the gatekeeper for inpatient hospital beds.</li><li>Conduct initial in-person follow-up after hospital discharge.</li></ul>  <h3>Main duties of the job</h3>
  <ul><li>Provide responsive, person-centred support to individuals experiencing mental health crisis in the community.</li><li>Work collaboratively with multidisciplinary teams, including clinicians, social workers, and peer support workers.</li><li>Deliver practical and emotional support to service users, promoting recovery, safety, and wellbeing.</li><li>Assist in crisis de-escalation and safety planning, both face-to-face and via telephone support.</li><li>Support service users to access appropriate services, including housing, benefits, and community resources.</li><li>Maintain accurate and timely records in line with Trust policies and procedures.</li><li>Participate in team meetings, supervision, and training to ensure high-quality service delivery.</li><li>Promote equality, diversity, and inclusion in all aspects of care and support.</li><li>Work flexibly, including evenings and weekends, to meet the needs of the crisis pathway.</li></ul><p></p>  <h3>About us</h3>
